My Front Teeth Don’t Bite Together! Before and After Open Bite Orthodontic Treatment and Review

This patient visited the clinic with the complaint that there was a gap between the front teeth, they did not bite together, and the mouth protruded. Also, as can be seen in the photos above, the lower teeth were rotated to the right (left in the photo), so there was a large difference in the midline between the upper and lower teeth. Due to the midline discrepancy, the bite pattern on the left and right molars was also different.

Because of the protrusion, the patient had to strain when closing the lips, which caused wrinkles to form in the chin area, and it was difficult to close the lips completely. In addition, due to slight asymmetry from the lower jaw being twisted to the right and differences in muscle activation at both corners of the mouth, the left corner of the mouth was drooping downward.

To improve the open bite and the difference in the dental midline, orthodontic treatment was carried out after extracting three premolars in the remaining areas except the lower right side.

After orthodontic treatment, the dental midlines matched, and the open bite where the upper and lower front teeth had been separated was improved. The degree of protrusion also improved significantly, and the lips that had protruded beyond the E-line (esthetic line) connecting the tip of the nose and the chin came to rest on the line. The left corner of the mouth, which had been drooping downward, also improved a lot compared to before treatment (the patient also practiced consciously lifting the left corner of the mouth while smiling).

As the protrusion and open bite improved, mouth breathing, which had been a habit of breathing through the mouth rather than the nose, also improved. Most importantly, after the orthodontic treatment was completed, the patient was very satisfied because they were able to bite through noodles with their front teeth.
