
Anterior esthetic prosthetics: Yonsei Garden Dental Clinic

First visit on 2025-11-28: Yonsei Garden Dental Clinic
This patient was a man in his early 40s from the United Kingdom. He said that, because he was born with a small jaw, he underwent orthognathic surgery to move the jaw forward in childhood, along with orthodontic treatment for about six years. However, the orthodontic treatment was not fully satisfactory, and later he injured his front teeth in an accident and received prosthetic treatment. Over time, his gums receded, exposing the metal portion of the front prosthetics, and he had been living for a long time with an esthetically unrefined condition due to Class II malocclusion and irregular tooth alignment. Wanting to improve the appearance of his front teeth, he came all the way from the United Kingdom to Korea.

As a result of the diagnosis, the patient showed a very deep bite, with the lower anterior teeth barely visible during occlusion. The front teeth that had been injured in childhood also showed extensive inflammation around the root area, making long-term preservation difficult. We therefore decided to extract the two front teeth, and considering the patient’s relatively young age, we considered implants in the anterior area in order to preserve as much tooth structure as possible. However, because the bone loss in that area was severe and the patient was a foreigner who could not make frequent long-term visits, we planned bridge prosthetic treatment that could restore function and esthetics in a relatively short period rather than implant treatment with extensive bone grafting.
In addition, because the lower anterior teeth were excessively tipped toward the tongue, there were limitations to restoring their position prosthetically. So after extraction, we established a treatment plan using bridge prosthetics for both the upper and lower jaws. If the lower anterior teeth were moved back to their original position, the bone was too prominent, so we also planned bone contouring in the lower front tooth area after extraction.
When carrying out extensive anterior restoration like this, it is essential to have sufficient consultation with the patient before treatment. The treatment plan may vary depending on the patient’s sex, age, and preferences, and through enough prior consultation and prediction of potential risk factors, we aim to achieve prosthetic results that leave the patient as satisfied as possible.
Therefore, on the first day, we only carried out various clinical photo records for diagnosis, model fabrication, and consultation. After that, we showed the patient an intuitive design using a diagnostic model created with 3D modeling. This makes communication smoother than vague explanations, and the patient can directly check the model and communicate the desired design more clearly.
Based on the diagnostic model, we held the first design consultation. After that, we performed root canal treatment on the teeth that would require substantial reduction for the bridge prosthesis, and made initial temporary prosthetics. We then placed these temporary teeth in the mouth and conducted another consultation. This is because the impression from looking at the model can differ somewhat from wearing it inside the mouth. In this process, it is essential to carefully check whether it harmonizes with the patient’s face and smile, and whether there are any discomforts with speech or occlusion. Although this patient was a foreigner, he was very proactive about treatment, and fortunately, he was able to use the temporary teeth for an extended period. After observing them for 1 to 2 weeks of actual use, we held a final consultation on the design and fabricated the final prosthetics.
One of the disadvantages of prosthetics is that, as time passes and the gums recede, spaces can form under the artificial teeth, making food accumulation more likely. However, if prosthetic treatment is carried out immediately after extraction, the artificial teeth can be made to fit the extraction sites, so even if the gums later recede, the artificial teeth remain positioned deeply in the gums, much like natural teeth. In this case as well, we took advantage of immediate restoration after extraction and made all the artificial teeth deeply positioned to minimize problems.

After the treatment was completed, the patient’s smile overall improved into a more stable and harmonious impression compared with before. The length, shape, and alignment of the front teeth naturally harmonized with the smile arc formed along the curve of the lower lip, allowing a soft esthetic balance to be restored without excessive tension when smiling. Along with functional occlusal stability, esthetic satisfaction was also high, and the patient was very pleased with the result.

Anterior esthetic prosthetics are not simply treatments to improve tooth shape; they are treatments that must consider the harmony of the entire face and smile. In this case, from the diagnostic stage through verification with temporary prosthetics to the fabrication of the final prosthetics, the design was carefully adjusted based on thorough communication with the patient. As a result, treatment was completed with predictable and stable outcomes in both function and esthetics. This was a case that once again showed how important a plan that accurately reflects each patient’s condition and needs, along with sufficient communication, is in esthetic prosthetic treatment.
