As a result of the detailed examination, the mandibular dentition was deviated to the left, which caused the upper and lower dental midlines not to match. The lower dentition was positioned overall forward, making the lower lip protrusion appear more pronounced. In addition, the maxillary arch was narrower than the mandibular arch, and a partial anterior crossbite was observed in the front tooth area. In particular, in the case of traumatized teeth, there is a risk of additional inflammatory root resorption when orthodontic force is applied, so continuous observation during treatment was essential.

After the examination, orthodontic treatment was carried out according to the following plan.

• Extract the lower wisdom teeth

• Use the extraction space to move the mandibular dentition to the right

• During treatment, regularly take front-tooth X-rays to continuously monitor whether root resorption occurs in the traumatized tooth

In the photo taken 7 months after treatment start, after aligning the teeth, a mini-screw was placed on the lower right side to improve the dental midline, and it can be seen that the occlusion and dental midline have improved significantly.

Over about one year of orthodontic treatment, the misaligned dental midline was naturally corrected, the midline asymmetry improved, and the lower lip protrusion also improved noticeably. Above all, during the treatment period, the root resorption of the front teeth remained stable without any further progression.

We recommended further improvement of the protrusion, but since the patient suddenly enlisted in the military, we decided to finish treatment in the current state. Before and after treatment, the upper and lower dental midlines matched, and the left-right molar occlusion also improved. With the posterior movement of the mandibular dentition, the degree of lower lip protrusion also improved greatly.
