I’m Baek Won-seon, the chief dentist at Lev Dental Clinic near Sinsa Station.

This is a post for those considering front tooth implants.

There usually aren’t many cases where front teeth need implants, but there seem to be two main types.

1. In cases where a front tooth is broken due to decay or trauma.

2. In cases where a front tooth is loose and pus is coming out due to chronic periodontitis.

I’ve previously covered a case where the crown of a front tooth broke off together with the root,

and I’ve also had cases where the periodontitis was so severe that we extracted the tooth and resolved everything neatly with a bridge.

All of them were valuable cases!

Broken front tooth.

A front tooth that had undergone root canal treatment due to secondary decay broke.

This patient came in with the upper right central incisor broken.

They had delayed visiting the clinic for quite a long time because of cost concerns.

If you look at the dental panoramic image, you can see that only a very short root remains..

If you look from the side, you can see several teeth that are down to just the root...

and several teeth with decay at the tooth neck area (cervical area).

We extracted it and placed the implant right away.

We also performed a small amount of bone grafting.

Because the remaining tooth was small, we were able to place the implant immediately while removing the broken tooth.

We filled the space between the extracted site and the implant with a small amount of bone graft material.

You can see that it was placed in the proper position.

After 3 months, the new jawbone and the implant will achieve osseointegration.

If you look closely, you can see that the gum has healed neatly around the implant, right?

And there was one more change! During the 3-month healing period, we treated the decay at the tooth neck area neatly as well!

It does show a little bit, but the patient was reportedly very satisfied.

When you come to our dental clinic near Sinsa Station, we have an in-house lab, so in the case of front tooth implants, the dental technician in charge personally consults with you about the color and shape.

In any case, being able to speak directly with the person making your dental prosthesis is a big advantage, isn’t it?

Striped teeth that were more difficult than expected to match in appearance.

There was a little regret, but the patient was fairly satisfied.

From the back, it stands out quite a lot. Haha. We temporarily cement it like this and let you use it for a week.

You can also see the properly made implant crown on the panoramic X-ray.

Neat and tidy~~ It’s not enough to just place the implant; the crown has to be made well for everything to be complete!

Loose left canine and front tooth with pus.

This patient said pus was coming from the upper left canine (front tooth).

If you look, you can see pus coming up between the tooth and the gum even while they are just sitting still..

They were a smoker, and the chronic periodontitis had worsened into a periodontal abscess.

Even on the dental panoramic image, you can see that the jawbone around the upper left canine has disappeared,

And if you look at the periapical radiograph, which can be magnified, you can see that the bone around the canine root has become dark as it has been lost.

A significant amount of gum bone had been destroyed by chronic periodontitis.

In areas where gum inflammation is this severe....

it is more difficult than expected to place an implant immediately after extraction.

Because once the tooth is extracted, there is no bone left.. and there is a lot of pus around it...

In such cases, we recommend extracting the tooth and waiting about 1 month before placing the implant.

By then, the signs of inflammation will have disappeared, and the gums will have healed firmly.

So we extracted the two front teeth that had been very loose,

and placed two implants one month later.

Extensive bone grafting as well!

In cases like this, where the front tooth was extracted because of gum inflammation, even if you wait a month and then place the implant, quite extensive bone grafting is needed. This is somewhat different from the previous case, where only the root remained but the surrounding bone was healthy.

As expected, we carefully measured the tooth color using a shade guide to determine the shade.

If you look closely at adults’ teeth, you’ll see that they are more difficult to match in color than you might expect. ^^;;;

So this is the front tooth we made to match as closely as possible.

We created a two-unit prosthesis combining the upper left lateral incisor and canine!

In the dental radiograph taken after completion, you can also see the neatly finished prosthesis.

It’s not enough to just extract a tooth and place an implant immediately.

To get a good result, you need a good treatment plan, right?

While the overall treatment time can be shortened.. the best condition for placing an implant while extracting a tooth is whether there is healthy jawbone. And the dentist has to be able to recognize that, of course.

The first broken front tooth, where only the root remained, was exactly that kind of case. Even though there was only the root left, the surrounding jawbone was in very healthy condition. In such a case, placing the implant with a simple bone graft is a very good case indeed—shorter treatment time and a better outcome!

In the second patient, where pus was coming from the gums and the tooth was loose, the pus had been present long enough to cause such extensive destruction of the jawbone around the tooth that placing an implant while extracting the tooth was difficult.

In such cases, people often end up forcing the implant placement and placing it far too deep.... Then the trouble starts once the crown is completed. Please be careful.

There are so many factors to consider in order to make one tooth beautiful: the condition of your own teeth, gum disease, the state of the surrounding teeth, age, health condition, and more... There are far more things to consider than you might think.
