Our clinic has now been open for about three months. At first, I wondered whether we would have many implant patients, but now it feels like we are becoming a mecca for teeth whitening and laminates.

One of the treatments our clinic is focusing on and working hard at is Reub Dental Clinic laminate; Reubnate.

Today, I’ll give a brief explanation of teeth grinding and clenching, and briefly explain the treatments for them.

Teeth grinding = Bruxism

Clenching = Clenching

The mechanism of teeth grinding is said not to have been clearly identified.

Since stress is the root of all illness, it is only being presumed that stress is the cause.

Teeth grinding in young children usually disappears as they grow into adulthood,

but teeth grinding in adults is mainly severe during sleep (at night, of course), and clenching is often found during the day as well.

The Masseter shown in the picture above is a muscle that connects the cheekbone and the lower jaw. When you clench your teeth tightly, it protrudes from the cheek area and can be felt; it is the main muscle used to close the mouth.

The Masseter is the green muscle shown above, and the second largest muscle involved in closing the mouth is the Temporalis, which is the pink muscle shown above.

As expected, when muscles act, they shorten and bring the lower jaw closer to the upper jaw. (That is what closing the mouth is, I suppose?)

Normally, the natural position of the lips should be slightly apart like this.

At the dental hospital of my alma mater, Yonsei University College of Dentistry, Oral Medicine, this was called the position when pronouncing the letter ‘N,’ so it was called the N position.

But depending on the person, there are people who keep their mouths tightly closed when concentrating on work or exercising, and so on.

Going further, they may even clench their teeth tightly. Then this becomes clenching.

If you keep your mouth closed too tightly, excessive pressure is applied to the temporomandibular joint between the lower jawbone and the skull.

This can cause changes in the cartilage between the two bones (the jaw disc).

Of course, it doesn’t happen overnight. It has to accumulate over time.

As a result, at first there may be some clicking in the jaw... then some pain... and if it progresses further, you can’t even open your mouth.. things like that.

Sometimes, of course, people also clench their teeth tightly while sleeping. That is also clenching. But if it becomes more severe, they begin grinding their teeth. Then it becomes teeth grinding, or bruxism.

The result, as explained above, is deformation of the jaw disc and the resulting pain or limited mouth opening (difficulty opening the mouth).

Furthermore, the teeth begin to wear down and break, leading to fractures. All of this happens because the Masseter and Temporalis are used excessively.

Let’s be careful about clenching during the day. Let’s try changing the habit. Even so, if you consciously let your lower jaw relax slightly during the day to create the ‘N’ position, it can greatly help your jaw joint and dental health. Of course, it won’t be easy!.....

Or you can make a soft guard like this. Wouldn’t you say it’s a slightly smaller version than the mouth guard we commonly know?

Usually, a dental clinic can make one for around 100,000 won. It doesn’t take long either.

However, I recommend removing it when you sleep. ㅎㅎ If you clench too hard or grind your teeth, it will quickly get holes and tear.

There really isn’t a magical fix for teeth grinding. ㅠㅠ That is why bruxism Botox and a bruxism appliance are needed. Since teeth grinding usually happens while sleeping, there really isn’t a perfect solution. Even if medication is used, there is no guarantee it will work. That said, what use is it to simply tell modern people to live stress-free...

That is why bruxism Botox treatment is recommended.

Usually, depending on the person, about 25 units per side are injected. We place them in 2–3 points. For people with severe bruxism, more may be needed. (If you search the internet for Botox dumplings, you’ll find it.)

If you receive bruxism Botox like this about four times at 3–4 month intervals (so it takes a little over a year), the masseter itself decreases in volume and strength, making it difficult to grind your teeth (?!)

Also, the aforementioned temporalis is not injected in all bruxism patients, but is selectively treated with bruxism Botox in severe cases. Usually, we treat the masseter first, then reassess after 1–2 weeks and inject the temporalis.

If you inject both the masseter and temporalis all at once without reassessment, you may become too weak to eat.

We also make a bruxism appliance.

The appliance referred to here is different from the soft guard mentioned earlier; it is a hard resin-made ‘resin splint.’ It is a fairly rigid device made from an impression taken inside the patient’s mouth. It usually costs around 500,000 won.

First of all, if you wear the bruxism appliance while sleeping, it helps protect the temporomandibular joint from excessive force, which is beneficial for jaw disc health.

In some cases, it can also guide the jaw into a position where it can rest a little.

And even if you grind your teeth, the resin splint contacts them instead of the teeth, so the teeth will not break. However, since the device is made of resin, it will wear down... so it needs to be repaired during regular checkups.

Therefore, a characteristic of this treatment is that you need to come in periodically so we can check whether the appliance has worn down and adjust its height if necessary.

Reub Dental Clinic at Sinsa Station. What efforts are being made for successful bruxism Botox treatment?

One of the most expensive materials among bruxism Botox products is ‘Xeomin.’

If you’ve looked into bruxism Botox at all, there’s probably no one who doesn’t know about it. It is a German product, and the cost price of purchasing 100 units is in the hundreds of thousands of won. To exaggerate a little... it is more than 10 times more expensive than cheap domestic products.

The reason it is expensive is that it undergoes an additional purification process to create a Botox formulation with less resistance. They say it is also more effective. To use an analogy, it leaves only the ingredients truly needed for the effect and removes the rest as much as possible... so the molecular weight is also smaller.

The domestic version of Xeomin is ‘Coretox,’ sold by Medytox. (The one used at Reub Dental Clinic at Sinsa Station.) It is the best among products from domestic companies, and it is about 4–5 times more expensive than Botox sold by smaller pharmaceutical companies. It is really quite expensive. ㅎㅎ Still, its effect cannot be compared with aesthetic-use Botox. Some even say it is better than Xeomin...

If you look at the news, you’ll realize just how many Botox companies there are.

In dentistry, Botox is used for therapeutic purposes, not cosmetic ones, so a higher dose is used. And because it is for treatment, there is no value-added tax that applies to cosmetic treatments.

Because the focus is on resolving the bruxism itself, you absolutely need to use a Botox product with strong performance. (If the injection doesn’t eliminate the bruxism, that would be a problem.) And who would know the muscles that move the mouth better than a dentist? Understanding temporomandibular joint disorders, diagnosing them, and observing and analyzing how they affect the teeth—dentists are the most suitable for that. Dentists are people who give anesthesia injections throughout their lives, and they are fated to always inject without causing pain. For these reasons, it is good to receive bruxism Botox treatment at a dental clinic!!

Botox is said to have a significant effect on permanently reducing muscle ability when administered four to five times at 3–6 month intervals. After receiving bruxism Botox, you may feel a noticeable reduction in jaw strength within 1–2 weeks, and you may also notice that your face becomes slimmer. It may be inconvenient, but if you receive it regularly, it helps greatly in permanently reducing the jaw muscles. (We send reservation reminder messages from the clinic!! :)

If you have had jaw Botox for cosmetic purposes, you may have received a smaller dose than what is used for bruxism patients. Also, there are reports that using a cheaper product clearly reduces the effect.

If you have jaw joint pain because your bruxism or clenching is severe, and if you have experienced broken or painful teeth, why not visit a dental clinic this time and get examined? Bruxism Botox is an effective treatment, but if you use a bruxism appliance well as well, you can become free from jaw joint pain and tooth pain!!
