
Today, I will show you a full-mouth implant case.
She was a young woman, but she lost her teeth early.
If you live for a long time with missing teeth,
the alveolar bone that supported the teeth gradually
disappears, and the shape of the face changes a lot.
As the alveolar bone melts away like this,
when the facial shape changes significantly,
restoring it to how it was before
becomes extremely difficult.
Because this patient was a young woman,
the most important part of the treatment process was
aesthetic satisfaction.
After the full-mouth implant treatment, shaping the face
beautifully was a very, very important part.
However, there was already a significant loss of alveolar bone,
so there were many difficulties in planning the full-mouth implant
treatment.

If you look at the side-view X-ray, you can see that the angle of
the upper lip has curled inward a lot (yellow arrow).
When the upper jaw bone melts and disappears, resorption occurs
in the direction of the arrow, and as a result, the upper lip curls
inward, the lips become thinner, and the area under the nose
changes into a face that looks sunken.
In full-mouth implant treatment, where there are major changes in
the facial bones, the most important thing is the temporary denture.
You may wonder, "I came for implant treatment, so why a denture?"
but one of the most important things in implant treatment is the
position where the implant is placed.
If the position is wrong, you might think it is no big deal at first,
but later a problem may arise that leaves you unable to do anything
about it, so caution is needed.
Isn't it enough to simply place the implants well in the center of
the bone?
Absolutely not!
Especially when there are fewer remaining teeth, the position and
angle at which the implants are placed are
important enough to be emphasized 10,000 times.
In particular, it is essential to make a temporary denture while
considering that the facial shape will also change, and to predict in
advance:
- The length of the face
- The support pattern of the lips
- The angle of the front teeth
- The implant placement position
Although 3D technology has advanced and virtual predictions are
possible even without a temporary denture, when there are no teeth
at all like this, there are no reference points, so you should not rely
only on 3D technology.
You must make a temporary denture to determine the final tooth
position.
Since I also provide orthodontic treatment, when carrying out this
type of treatment I use a facial analysis program used in the
orthodontic department.
Using these tools helps with understanding the basic skeletal form
and making a treatment plan.
The temporary denture is adjusted to determine the final treatment
goal.
And the surgery is performed to match that goal.
Before digital dental technology became widely developed, everything
depended on the dentist's intuition and ability.
These days, digital technology has improved a lot, and when used
properly, it helps us achieve the goals we planned more accurately.

Even if you decide, "Let's place it here and here," it is really
difficult to actually place it exactly in that position during surgery.
However, digital technology makes not only full-mouth implants but
also all dental treatments easier and faster.
But!
To apply this kind of technology in my dental clinic,
there are inevitably troublesome and difficult tasks such as
investment costs and staff training.

To be honest, introducing it at every dental clinic requires quite a
lot of investment.
Because of that, these technologies are used mostly by large dental
clinics.

Areumdri Dental Clinic is not a large clinic,
but because I have a personality that cannot stand falling behind...ㅠㅠ
I always end up investing a lot in equipment and technology.
Switching to a large clinic, hiring salaried doctors, and running it
with lower prices is also an option,
but I still cannot let go of the desire to check every patient
from start to finish with my own eyes and take responsibility.
That is why I have not moved to a large clinic and am still working
hard on my own at Areumdri Dental Clinic.

Let me show you the patient's photos before full-mouth implant

In the photo below, the white part is bone.
Originally, there should have been bone up to about the green line,
but if you stay for a long time after tooth extraction, the bone is
resorbed and the remaining bone becomes that thin, as shown in the
photo.

Placing implants in such thin bone is not easy.
Rather than focusing only on placing the implants, I also consider
long-term maintenance and management, and no matter how good the
grafted bone is, it is not as good as my own bone.
In addition, because the patient was young,
I judged that treatment should be done so that it could be used for a
long time without problems.

This area still has some bone.
However, even though there is a little space front to back,
the left/right space is very narrow!
When there is no bone, there are many limitations in the implant
placement angle,
and if you place it thinking only about the bone, there may be
problematic situations later when raising the crown part,
so it is better to check these things in advance before placing the
implants.
This way, you will not regret it later when you have to raise the
crown after the implants are already placed.

The full-mouth implant placement and all treatment through the
crown have been completed successfully.
I still vividly remember how she cried because of the missing teeth
when she first came to consult at Areumdri Dental Clinic.
But!
The sight of her being truly, truly happy after the full-mouth
implant treatment was completed successfully is even more vivid.
She still comes in for regular checkups, and although she used to be
so shy, every time she comes she says, "Doctor~~!" and hugs me.^^
Moments like this are really rewarding.
