Amdureu Dental Clinic Treatment Diary: Before and After Implant Esthetic Prosthetics

Hello. I’m Lee Hyun-jung, director of Amdureu Dental Clinic.

A patient came to see us from far away in Gyeongsangnam-do.

She was an older woman,

and she said she had a presentation a few months later and wanted to look nice, so she came to see me from far away.

Before coming to see me, she had already had the teeth with significant mobility extracted.

So there were gaps where teeth were missing.

I personally dislike having the front teeth spaced out and missing like this, so if it were me, I would have extracted them and immediately placed implants and made the teeth right away...

But she had already been left in that state, and it made me feel sorry.

Still, since she came from far away saying she wanted to look pretty, we had to find a way to treat her beautifully~

There are gaps from extractions here and there. It would have been better if the implants had been placed right away during extraction, which is unfortunate.

The remaining teeth that had not been extracted were also in poor condition, and several implants had already been placed in the molars.

But the right molars were not biting properlyㅠㅠ

It must have been very inconvenient for her to eat...,

The patient was very easygoing, and because eating during implant treatment is so difficult, by the time implant treatment is finished, there are frequent cases where the patient does not realize that the treatment has left them unable to chew properly and just moves on.

Still, it is better than how it was at the start of treatment...

But since the purpose of implant treatment is so clear, it is best for the doctor to properly adjust the biting surfaces on their own.

The bone in the area of the teeth that were not extracted protrudes a lot. (blue line) On the other hand, the bone in the extracted areas has sunken in. (yellow line) The two front teeth are sticking out. The neighboring tooth was extracted and the bone was resorbed, so it looks relatively more protruded.

When I asked the patient, she said her mouth had looked protruded since she was younger.

I did not want to simply place implants; I also wanted to correct the protruding mouth shape beautifully.

The lower side also shows a sunken appearance where teeth were extracted. If implants are placed immediately after extraction, this bone resorption can be reduced at least a little!

This is the result of incorrectly calculating the implant positions on the upper and lower right sides.

The very back tooth on the upper side does not need to be placed because there is no counterpart on the lower side.

Moreover, because the upper and lower bite had not been adjusted, only one point was actually contacting, while the rest was floating up.

The food must not have chewed well...

The patient said she had not known this all along.

In cases like this, the treatment had been done at another clinic, but... Since this is just the way I am, I can’t just leave something like this alone... I wanted to correct that area.

Moreover, people with protruding mouths often cannot keep their lips closed well in daily life. When the lips close properly, the mouth does not dry out, pronunciation improves, and it becomes more comfortable. My goal was to fix all of these things.

Treatment goals

1. Make it so that food can be chewed properly
2. Make the protruding mouth look less protruded
3. Make it easier to keep the lips closed

The angle of the front teeth is very flared. The molar bite (green) had not been properly adjusted, so the upper and lower teeth were visibly floating apart!

I reduced the angle of the front teeth so the mouth would look less protruded. I adjusted the molar bite so food could be chewed properly, and I slightly shortened the facial height and adjusted the front tooth angle so that the lips would close comfortably.

Because the patient was older, if we shortened the facial height too much, the cheeks could look saggy, so we only reduced it slightly.

There is no need to place implants in every single missing tooth.

It is best for the patient, too, to place only the minimum number of implants by selecting only the positions that are truly necessary!

Seeing her smiling brightly makes me happy too. She came all the way from far away, taking the early morning train and working hard to visit us, bringing warm rice cakes and corn from early in the morning...

I remember treating her with a heart full of gratitude every time.

I believe she was able to finish her presentation well looking beautiful.
