Checking the Treatment Duration for Clear Aligner Invisalign Correction of Crowded Canines

Today, I’ll introduce the extraction orthodontic treatment process with Invisalign in cases of severe crowded canines.

There are two reasons for extraction in orthodontic treatment.

Severe crowded canines,

or severe protrusion.

Although both cases involve the same extraction orthodontic treatment, when treating with Invisalign, everything is different, including the treatment plan, attachment placement, and the position and direction of the elastics.

Today, I’ll explain a case of extraction orthodontic treatment due to crowded canines.

This patient visited us in the fall of 2022 for correction of crowded canines.

The canine crowding is severe. This is called a high canine.

Because the crowding is severe, extraction of four premolars was unavoidable.

We also decided to address a slight lip protrusion.

Treatment with Invisalign began, and after waiting a month and a half for the aligners to be fabricated, the patient started wearing Invisalign aligner No. 1 in October.

This is how it looked 11 months after treatment began.

The canines are descending well into the extraction space.

In January 2024, we began remaking the appliances to improve the tilted axis of the premolars, and by November the first set of additional appliances had been fully worn.

The molar occlusion was still not fully satisfactory, so we remade them once more.

The round parts attached to the molars are hooks for elastic bands.

The alignment is good.

After wearing the second set of additional appliances for four months from December 2024 to April 2025, treatment was completed.

The basis of orthodontic treatment is a tight, even occlusion.

If the occlusion is not good, treatment is not finished.

The patient did not want a wire retainer because of concerns about discomfort. We decided to maintain the result only with Invisalign clear retainers, Vivera, and fabricated only the Vivera retainers.

Now, let’s look at the before-and-after comparison.

There were a total of two remakes, and the treatment period was 2 years and 6 months. Excluding the periods when appliances were being fabricated in between, the actual treatment period was 2 years and 2 months.
