Today I’ll introduce an orthodontic case in my 40s.
This is an Invisalign Lite case in my 40s.
You may think clear aligners like Invisalign are not cheap, so of course they must cost more than traditional braces, but if you use Invisalign Lite, a partial orthodontic system for simple malocclusion, wisely, you can get Invisalign treatment at the cost of Clippy C braces.

Today I’ll talk about how we used Invisalign Lite to upright a tipped molar, resolve the deep bite, and neatly correct the crowded tooth.
It will also be helpful if you see how orthodontic treatment progresses when there is a supernumerary tooth.

This patient came in for orthodontic treatment a year ago.
They had originally been accompanying a family member for an orthodontic consultation, and ended up starting their own consultation by chance.
You can see midline discrepancy, severe deep bite, and crowded canine teeth.

They had been thinking they would get implants someday, but time passed with the teeth missing, and the surrounding molars tipped into the empty spaces.

In particular, in the blue-arrow space, the back wisdom tooth had tipped forward quite a lot, making implant placement difficult.
Orthodontic treatment to organize the surrounding teeth that have tipped and come down in order to recover this narrowed implant space is called prosthetic orthodontics.

The narrowed space and the tipped wisdom tooth can be confirmed on the panoramic X-ray.
In cases like this, removing the wisdom tooth would solve the problem simply, but there are already many teeth that have decayed and been lost, and the wisdom tooth has erupted well and has good contact with the opposing tooth, so we decided to preserve the wisdom tooth as much as possible and use it.
There is one more issue confirmed in the initial detailed diagnosis.
It is a supernumerary tooth between the front teeth. The supernumerary tooth located exactly in the center is called a mesiodens.
After taking a CT scan, we found that the supernumerary tooth was close to the nerve canal and deep, making extraction difficult.
The good news is that if we minimize movement of the front teeth, orthodontic treatment can still be done without having to remove the supernumerary tooth.
Considering that we decided to preserve the wisdom tooth, that movement of the front teeth would be limited because of the supernumerary tooth, and finally that the patient preferred treatment that was as unobtrusive and simple as possible, we set the treatment goals.
We explained that if the wisdom tooth was not extracted, it would be difficult to achieve a proper Class I relationship on the right side and the midline would not align exactly, and we planned to do a short course of treatment with Invisalign Lite.
Invisalign Lite provides up to 14 aligners.

The patient wore the aligners diligently.
Let’s look at the changes after wearing all 14 aligners over 6 months, from May 2024 to November 2024.

We uprighting the tipped molar and secured about 2 mm more space for the narrowed implant site.
The alignment also improved a lot.
Although things improved a lot from the beginning, we proceeded with a remade set to secure a bit more implant space and increase the completeness of the alignment.
From December 2024 to March 2025, the additional aligners were all worn over 4 months, and the orthodontic treatment was completed.
After that, it took another 3 months to place the implant and complete the implant prosthesis, and in June 2025 we finally finished both the orthodontic and implant treatments.
From the point when orthodontic treatment ended until the implant prosthesis was completed, the clear retainer was worn only during sleep for 3 months to maintain the orthodontic result.
Let’s look at the result.

There was no root resorption, and root parallelism was good.
The implants were placed beautifully, and the wisdom tooth occlusion is also good.
