Invisalign Extraction Orthodontic Treatment Review for a Woman in Her 30s with Protruding Teeth and Crowding

Today’s post is about Invisalign extraction orthodontic treatment for protruding teeth.

This is a patient in her 30s who visited for orthodontic treatment in January 2023.

What she wanted to correct was her protruding teeth, and she chose Invisalign for the appliance.

Crowding of the front teeth is visible.

The midline is off by about 2 mm.

The protrusion of the front teeth is quite severe.

Because she wanted Invisalign, we recommended Invisalign extraction orthodontic treatment.

We decided to extract four premolars.

A total of 80 aligners were made for the first set.

The treatment was carried out by changing to one new aligner every week.

About half of the extraction space had closed,

and the treatment was progressing well without any lifting or poor fit of the aligners.

Finally, all 80 aligners had been worn by August 2024. It had been 1 year and 8 months since treatment began.

The midline is well aligned, and the front teeth bite is also good.

The extraction spaces look completely closed, but there are still tiny spaces left, enough for floss to slide in loosely.

To improve the unfinished areas, additional aligners were remade, and from September 2024 to April 2025, after wearing all 25 additional aligners, this is the result.

Everything was good, but the patient’s concern was that one of the left front teeth was tilted.

To address this, we decided to use additional aligners one more time. From May 2025 to July 2025, after wearing 10 more additional aligners, treatment was finally completed.

the molar bite is Class I, and the fit is tight with no gaps.

Now that the front tooth protrusion has been resolved, the lips that did not close comfortably now close naturally.

These days, when talking with patients preparing for orthodontic treatment, many worry that extraction orthodontic treatment will make the mouth look sunken in.
