Today, after a long time, I’d like to talk about Invisalign First, a clear aligner treatment for children.
This is the children’s clear aligner treatment often referred to as the “three-item set” for Gangnam moms, along with Dream Lens and growth hormone injections.

Since the launch of Invisalign First for children, orthodontic treatment has become much more convenient, and the results have improved.
In the past, children’s orthodontics often ended after only solving urgent issues such as simple arch expansion or correcting a crossbite, which meant that a second round of orthodontic treatment was often necessary in adulthood.
But Invisalign First has significantly reduced the likelihood of needing orthodontic treatment again later.
Of course, if the goal is to reach a level where retreatment is not needed later, simply aligning a few front teeth and doing a little arch expansion
and ending treatment that way does not greatly reduce the chance of needing orthodontics again later.
You need to make full use of the 18-month warranty period set for Invisalign First, and continuous monitoring of eruption is necessary so that the permanent teeth come in properly.
In addition, the design and timing of retainers must be planned strategically.
Only then can the chance of needing orthodontic treatment again later be minimized.

This is a 9-year-old child who visited for orthodontic treatment.
The parents had experience with Clippy-C braces orthodontic treatment and wanted their child to receive Invisalign First clear aligner treatment,
so they came to the clinic.
What the parents wanted to correct was the deep bite where the front teeth bite too deeply.
It is a severe deep bite to the extent that the lower front teeth are not visible.

There is protrusion of the front teeth,

and the front part of the arch is narrow.

The lower arch also needed expansion.
At this point, there was not enough space, so the front teeth came in crooked, and the canines that were about to erupt were likely to come in as crowded teeth due to lack of space.
I recommended Invisalign First, and the child started wearing the appliance in August 2023.

The first set consisted of a total of 21 aligners.
All 21 aligners were worn by February 2024, six months later.

The deep bite improved, the lower front teeth became visible, and the midline was well aligned.

The flaring and protrusion of the front teeth were corrected, and several retained primary teeth that were left in between then fell out.

As the narrow arch expanded, its shape became more established,

The crooked lower front teeth became aligned, and as space was created through arch expansion, the canines were able to erupt into the correct position.
Some doctors finish treatment at this point, but we continue follow-up until the warranty period ends without ending treatment early. If needed just before the warranty period ends, remaking a set of aligners makes it possible to include the newly erupted permanent teeth in the aligners and align them, which can achieve results comparable to adult orthodontic treatment.
After diligently wearing the first set and completing six months of treatment, the child rested from February 2024 to November 2024, wearing only the final stage aligners from the first set at night as retainers.
This is the waiting period for the permanent teeth to erupt.

Remaking began in November 2024, and the remade aligners started being worn from December 2024.

If retainers are worn properly, there will likely be no need for orthodontic treatment again in adulthood.
Rather than blaming the limitations of children’s orthodontics, my constant goal is to prevent the need for a second round of orthodontic treatment.

There is no root resorption, and root parallelism is good.
The child is currently wearing a removable retainer only at night. With regular follow-up care, continued monitoring is needed to make sure the results are maintained at least until growth is complete.
