Treatment Review: Non-Extraction Orthodontic Correction for Crowded Teeth, Invisalign Overbite Correction, and No-Prep Laminate Veneers

There is a prerequisite for no-prep laminate veneers.

The teeth need to be aligned.

Today, I will show you a case in which the patient underwent orthodontic treatment with Invisalign and then had their color and tooth shape improved with Jelami, without any tooth reduction.

This patient came in for orthodontic treatment in October 2023.

The teeth are small, and wear and chipping can be seen at the edges of the front teeth.

There is a deep overbite, where the upper teeth cover the lower teeth significantly, and the midlines are also misaligned.

The molar bite is in good condition.

Because the alignment of the upper front teeth is uneven, doing no-prep laminate veneers in this state would still require trimming the teeth, and the shape and translucency would not look very attractive.

The lower premolars are displaced inward. In other words, these are ectopically erupted teeth, or simply crowded teeth.

What the patient wanted to correct was that the upper teeth looked protruded.

After analysis, there was no actual protrusion of the upper teeth, but because the teeth were not aligned, there were areas that appeared prominent in the dental arch, and there was also a large amount of gum display when smiling. Based on this, we decided to begin with orthodontic treatment.

To resolve the crowded tooth issue, instead of extracting the premolars, we decided to gain space by moving the molars backward into the wisdom tooth space.

The first set consisted of 56 aligners.

A total of four orthodontic mini-screws were placed to help move the molars backward.

From October 2023 to January 2025, this is the condition after wearing all 56 aligners for 1 year and 3 months.

The overbite has been corrected and the front tooth alignment has improved, but the midline is still not ideal.

After 1 year and 3 months of treatment, the gummy smile and slight cantiing have improved.

Even though the philtrum became shorter due to philtrum reduction surgery during the orthodontic treatment period, the amount of gum exposure improved.

We will now proceed with making additional finishing aligners to improve the midline and close small spaces.

There are 10 additional aligners, which were worn for 3 months from February 2025 to May 2025.

The midline is well aligned, and the bite is also good.

Now we begin preparation for the no-prep laminate veneers.

Because the teeth are short, we first perform gum contouring.

After gum contouring, and following about ten days of recovery, once the gums have healed, no-prep laminate veneers are bonded to the six front teeth.

The milky translucency created by the minimal thickness is a strength of Jelami.

Now that the chipped and worn edges of the front teeth are covered, the result looks much cleaner.

The total treatment period was 1 year and 7 months, and one remaking was done.
