Today, I’ll explain orthodontic treatment for closing spaced teeth while maintaining the lip profile to help prevent a retrusive smile.

This patient in their 30s came in with spacing between the front teeth.

The amount of spacing is quite large.
If we simply close all of the space, the front teeth would be pulled far back.

The front teeth show an edge-to-edge bite or even a reverse bite.

As you can see in the patient’s initial lip profile, the front teeth should not be pulled back.
So the method is to bring the molars forward, or move them mesially, to close the space.
The patient wanted Clippy-C treatment and started treatment right away.

To pull the molars forward, four orthodontic mini-screws are needed.
This is what it looked like in April 2025 when the space was almost closed.


Only about 1 mm of space remained on the lower front teeth.
We continued treatment, and in September 2025 the space was closed, so treatment was completed.

The midline is well aligned, and black triangles have hardly formed.

The molar occlusion is good.
Now let’s look at the before-and-after comparison.
The total treatment period was 1 year and 4 months.

The contact between the teeth was finished tightly.

Even though a large amount of space was closed, the lip profile was well maintained.
I hope this post is helpful for those who hesitate to undergo orthodontic treatment out of concern that their lip profile may not look as good.
