Today, I’ll share another Invisalign extraction orthodontic treatment case for protrusive mouth.

This is a middle school patient who came in for orthodontic treatment in October 2023.

What the patient wanted to improve was the protrusive mouth.
There was also crowding and misaligned teeth, so extraction of four premolars was necessary.

The lower jaw was small, with a retruded chin, but it was already past the stage for growth orthodontics to encourage lower jaw growth.
We explained that even if only the protrusive mouth were improved, the chin would appear less retruded, and then began treatment.
The patient chose Invisalign, and the first set consisted of 88 aligners.

This was when the patient was wearing aligner No. 51.

About one-third of the extraction space remained, and the crowding had been resolved.

The alignment had improved a lot, but since some aligners did not fit well, we moved early to remanufacturing.
Unfortunately, the remaining 52 to 88 aligners had to be discarded.
After waiting one month for the aligners to be remade, the patient started wearing the second set in December 2024. The second set consisted of 16 aligners.
During treatment, there was a period of about four months when treatment was paused due to the patient’s personal circumstances.
At that time, to prevent the teeth from returning to their original positions, the aligners were worn only at night for maintenance, without making progress.
Then in September 2025, after wearing all 16 additional aligners, treatment was completed.

The midline is well aligned, and the front tooth bite is appropriate.

The molar occlusion is Class I and looks good.

In the meantime, all the way up to the second molars have erupted and been well aligned.

Now let’s look at the before-and-after comparison.
The total treatment period was 1 year and 11 months, or about 1 year and 9 months if we exclude the break in the middle.
The younger the patient is, the fewer variables there are in tooth movement and the smoother it is, so the overall orthodontic treatment period becomes shorter.

With the protrusive mouth resolved, the retruded chin appears improved.
Because the patient was still in the growth period, the lower jaw also grew forward in reality.

The front tooth angle is appropriate, and

The jawline that had looked retruded became more attractive.
During the treatment period, the lower jaw grew in a desirable way, so the retruded chin actually improved.

There was no root resorption during treatment, and root parallelism is good.
