This is Apgujeong Orthodontic Clinic.
This patient came to us after being diagnosed at another clinic with an extraction-based orthodontic treatment, even though non-extraction treatment was sufficient and there were no issues with facial profile.
Today, I will show a case in which crowding, the midline, and a scissors bite were improved using wisdom tooth space in non-extraction orthodontic treatment.

This patient came in for orthodontic treatment in February 2024.
What they wanted to fix:

What they wanted: maintain the current facial profile

The midline is off by about 3 mm.

Crowded teeth are visible, and there is also a scissors bite in the premolars indicated by the blue arrow.


The patient had already been told during an orthodontic consultation at another clinic that “if extraction is not done, the mouth will protrude more than before, so extraction is necessary.”
Because the crowding and protrusion were not severe enough to require extraction, we recommended a non-extraction approach: remove impacted wisdom teeth and use the wisdom tooth space to move the molars backward.
By April 2025, the patient had worn all 51 aligners in the first set.

The midline discrepancy remains, but the front teeth alignment is good.

The molar occlusion is also good.

To push the molars backward using the wisdom tooth space, an orthodontic screw was placed in the palate and treatment continued.

To further improve the midline and front tooth alignment, additional aligners were made.
From May to October 2025, the patient wore all 11 additional aligners.

The molar occlusion is a flawless Class I occlusal relationship.

The total treatment period was 1 year and 8 months, and the aligners were remade once.

The attractive facial profile was also well maintained.

There was no root resorption, and root parallelism was good.
