Today, let’s talk about Invisalign First for children.

Primary orthodontic treatment, in other words pediatric orthodontics, is like carrying out major work in advance before the second phase of treatment.
However, after the introduction of Invisalign First, a clear aligner system for children, the need for a second phase of treatment has decreased a lot.
Of course, it cannot be guaranteed 100%.
There are definitely cases where a slight deep bite recurs, where a premolar that will erupt later comes in rotated, or where the bite becomes slightly off due to limits in predicting jaw growth.
To prevent these issues as much as possible, if there is an underbite (Class III malocclusion) or an overbite/weak chin (Class II malocclusion), the timing should be delayed or treatment should be approached in line with the peak growth period, and the treatment period is set at nearly two years, extending beyond the 18-month warranty period for Invisalign First.
Of course, the appliance is not worn for the entire two years. There are also periods in between when it is not worn during the day while waiting for the permanent teeth to erupt.

This child came in for orthodontic treatment in October 2023.
There is a height difference between the two front teeth, and a deep bite where the front teeth overlap too much.


Among the four front teeth, there is not enough space for the lateral incisors, that is, the side front teeth.

The lower teeth are the same.
The front teeth are already crowded and crooked.
What will happen when the canines come in?
Crowding is 100% guaranteed.
Pediatric orthodontics involves expanding the arch to widen the space for the teeth to erupt.
We began Invisalign First treatment.
The first set consisted of 23 aligners.
After wearing all 23 aligners for five months, this is the result.

The deep bite improved, and because space for the lateral incisors was created in advance through arch expansion, the lateral incisors came in well.

To improve the midline discrepancy and secure a little more space for the canines to erupt, we proceeded with additional aligners.
The number of additional aligners was 19.
If children cooperate well with the aligners, they change them every five days.
After wearing all of the additional aligners for three months until August, this is the result.

The midline aligned well, and the deep bite improved further.



Now we enter a resting period.
This is the stage where we wait for the permanent teeth to erupt and monitor the progress while the warranty period is still remaining.

During the resting period, the last aligner worn is kept in only at night as a retainer while waiting for the permanent teeth to erupt further.
From August 2024 to February 2025, we took a nine-month break while wearing the retainer only at night, and then proceeded with the final remake.
Treatment was completed after wearing all 16 aligners of the second additional set.

To prevent the deep bite from recurring, the retainer with a bite plate is worn only during sleep.
