Invisalign Lite Correction for Peg Laterals / No-Prep Laminate Gelami

Today, I’d like to introduce a treatment case that included Invisalign Lite correction for peg lateral incisors and no-prep laminate Gelami.

The patient visited for orthodontic treatment because the front teeth were spaced apart.

The lateral incisors are peg laterals with a small size.

During orthodontic consultations,

“I think my front teeth are big.”

This is something we hear often.

However, when we actually look inside the mouth, the front teeth are often normal in size.

They only look that way because the alignment is not straight.

It only looks that way because the two central incisors are positioned slightly lower or protrude slightly forward. Teeth strictly follow perspective.

Even if they are only 1 mm farther forward than the adjacent teeth, they can look much larger than they really are. Conversely, if they are only 1 mm farther back, they cast a shadow and can look smaller than they really are.

We planned the treatment so that the spaced area would be corrected with orthodontics and Gelami could fit in beautifully.

In June 2023, the first set of 14 aligners was delivered, and the patient began wearing the first aligner.

About four months later, after wearing all 14 aligners, this is how it looked on the day we made the additional aligners in October 2023.

There is still a slight remaining space.

The blue arrow indicates the elastic hook used to improve the bite relationship.

As a mid-treatment check, we also checked whether the facial midline and dental midline were well aligned and whether the occlusal plane was tilted to either side before proceeding with the additional aligner fabrication.

The second set of aligners as additional aligners was delivered to the clinic in November 2023, and all treatment with the second set was completed in February 2024.

The additional aligner treatment period was 3 months.

This is the appearance immediately after orthodontic finishing and just before the no-prep laminate procedure.

The attachments, which are essential for Invisalign treatment, are still bonded to the teeth.

We finished the orthodontic treatment while leaving pre-calculated spaces on both sides of the peg lateral incisors.

These spaces are filled with no-prep laminate.

Both peg lateral incisors were treated in a balanced way with no-prep laminate Gelami.

This is the view from the occlusal surface after restoring both lateral incisors with no-prep laminate.

After the no-prep laminate is placed first, a wire retainer can be made.

Our orthodontic clinic follows the treatment philosophy that there is no need to become beautiful at the expense of your health.

No-prep laminate Gelami does not involve any tooth reduction at all.

Initial consultation: May 2023

Start of Invisalign treatment: June 2023 (first set)

Additional fabrication: October 2023

Start of additional treatment: November 2023 (second set)

Completion of orthodontic treatment: February 2024

Completion of no-prep laminate: March 2024

The total treatment period was 10 months, and the first set of 14 aligners was worn for 4 months.

The additional aligners were worn for only 3 months, so the actual orthodontic treatment period was 7 months.

The remaining 3 months were the waiting time for the aligners and the fabrication period for the no-prep laminate.

At first, the two central incisors clearly looked big, but after the alignment and height differences were corrected and the peg laterals were restored to a normal size with laminate, the central incisors no longer look big at all.

The spaced area was also neatly finished.

One concern many people have before Invisalign treatment is the bite.

The bite is also good.
