Among people who come in simply because they want to fix protrusion, many do not realize that a lot of gum shows when they smile.

We identify, diagnose, and treat even the issues patients may not have noticed themselves to create the most ideal result.

This patient in their 30s came in last April because of protrusion and crowded teeth.

They came from the provinces.

You can see deep overbite, crowded teeth, and a midline discrepancy.

The front teeth were misaligned.

In the profile analysis, the front tooth angle was normal and lip protrusion was not severe, so we decided to use the wisdom tooth space and aim for only a small amount of protrusion improvement.

Instead, to improve the gum display when smiling as well, we also planned treatment to intrude the front teeth.

Non-extraction protrusion correction is typically expected to take about 1 year and 6 months.

We chose and proceeded with Clippy-C correction.

Mini-screws were placed, and the entire dental arch is being pushed back into the wisdom tooth space.

Treatment was completed 1 year and 7 months after starting.

The midline matched precisely, and the deep overbite of the front teeth was resolved.

The two upper front teeth were large, so we refined the side shapes of the teeth with a minimal amount of interproximal reduction to improve the aesthetic finish.

Does interproximal reduction weaken the teeth?

People ask this a lot.

If the patient were to feel tooth sensitivity or weakening, we would have to take lifelong responsibility, but we do not perform a large amount of interproximal reduction.

If it is carried out only within the permitted range and to the degree of natural tooth wear, there is no discomfort at all.

As the amount of gum exposure decreased, the gummy smile improved a lot, right?

A slight buccal corridor was created, and a moderate buccal corridor improves the femininity and aesthetics of the smile.

The gum exposure improved with a proper front tooth angle that falls in a straight line, neither protruding nor slanting inward like buck teeth.

When patients look at the before-and-after photos, they are often amazed and say, “I didn’t realize my gums showed that much before.”

The lip position moved back by about 1–2 mm.

The root parallelism of the teeth is also good, and no side effects such as root resorption were observed.

Correcting a gummy smile and gum smile greatly enhances the completeness of the smile after treatment.
