Today, I’d like to introduce an Invisalign Lite case.
There are many cases where patients come in for retreatment in their 20s or 30s because they were not satisfied after non-extraction orthodontic treatment they received as children.
At the time of treatment, they may not have paid much attention to their appearance and simply wanted to fix crooked teeth, but as time passes, the parts they were not happy with begin to stand out.

This patient had non-extraction orthodontic treatment as a child 10 years ago.
The teeth were aligned and the molars fit well, but the reason for wanting retreatment was the protruding front teeth.

Orthodontic treatment is a process of using the space that is created to resolve malocclusion.
If there is not enough space, it has to be obtained somewhere, whether by extraction or by using wisdom tooth space.
If a patient wants crooked teeth straightened but does not want extraction or interproximal reduction, the treatment may end with protruded teeth.
When there is not enough space and the teeth are straightened, the teeth can end up protruding forward more than before.
Protruding front teeth are not only unattractive, but as people age, the gums covering the tooth roots are thin, so the gums recede more quickly. For that reason, it is better to avoid this situation.
For this patient, we decided to improve the protruding front teeth with Invisalign Lite along with interproximal reduction.
This was because the front teeth were fairly large to begin with, and on the apical X-ray, the enamel layer at the outermost part of the teeth was thicker than average, so a certain amount of interproximal reduction would not be a problem.
If the patient’s naturally sized front teeth had been small from the start and interproximal reduction had already been done during the previous orthodontic treatment, the doctor would not have recommended interproximal reduction.
To move the front teeth backward, we would have considered Invisalign Comprehensive, which is comparable to full orthodontic treatment that takes 1–2 years.
Interproximal reduction is a good tool for resolving malocclusion when applied in small amounts within physiological limits, but too much of it weakens the teeth.
This is the result after completing the first set of 14 aligners over 3 months.
The protrusion of the front teeth improved significantly, but in response to the patient’s request to move the front teeth back a little more, we proceeded with making additional aligners.

After waiting 1.5 months for the additional aligners, the patient started wearing them in October, and treatment was completed in January, about 3 months later.
Excluding the waiting time for the aligners, the actual treatment period was 3 months for the first set and 3 months for the additional set, for a total of 6 months.

If you compare the X-rays before and after treatment, the improvement in the angle of the front teeth is clear.
Even in retreatment cases or cases of protrusion, good results can be achieved with just 6 months of treatment depending on the patient’s condition.
