Every time I hear this question,
I can sense how worried patients are.
I understand that, too.
An implant itself can already feel like a burden,
and if bone grafting is needed as well, the cost goes up
and the scope of surgery seems larger, so it makes sense to worry.
So today,
I’d like to honestly explain whether bone grafting is really essential,
when it is needed,
and when it is not necessary.

- Bone grafting,
why is it said to be necessary?
An implant is a procedure that
places an artificial tooth root into the jawbone.
Just like when planting a tree,
its roots settle better when there is enough soil,
an implant also needs sufficient bone
to be firmly fixed.
A patient visited for molar implant treatment.
The upper right molar had already been extracted,
and because of maxillary sinus pneumatization, there was insufficient alveolar bone,
so we performed a sinus lift together.

The problem is that
when a tooth is left untreated for a long time
or when there has been gum disease,
the bone can be resorbed, becoming thinner
or lower in height.
If an implant is placed forcefully in this condition,
there is a risk that it may loosen or fall out later.

So when there is not enough bone,
we supplement it through bone grafting.
Simply put,
it is a process that creates an environment
in which the implant can last for a long time.

Here is an important point.
Bone grafting is not necessary for every patient.
Patients with good bone conditions
can still receive implants without bone grafting.
By taking a CT scan,
we can accurately check the width, height, and density of the bone,
which allows us to decide whether bone grafting is needed.

In some dental clinics,
bone grafting is reportedly included as part of a basic package.
But I believe it should only be recommended
for the areas where it is truly needed.
Recommending bone grafting to someone who does not actually need it
only increases the burden on the patient.

When we show patients their bone condition directly during the diagnosis process
and explain it to them,
most people say, “Oh, so that’s why it’s needed,” or
“This means I didn’t need it after all,”
and come to understand it.

please remember just one thing.
Whether bone grafting is needed or not
should be decided only after a detailed diagnosis.

Based on the CT scan results,
check whether the clinic clearly explains
why it is needed,
or why it is not necessary.
Treatment should proceed only after the patient understands and agrees,
so there is no regret later.

When I consult patients,
I also show them their bone condition directly
and explain specifically,
“This level is possible without bone grafting,”
or “This area needs reinforcement.”
