Forced immediate placement can instead lead to implant surgery failure and bone loss.

There are
two methods for implant placement.
Implant surgery
The method of placing implants is broadly divided into delayed placement and immediate placement (one-day implants).
General delayed placement
Extraction → gum and bone recovery → placement
This is a method in which the implant is placed after the gums and bone have healed to some extent following extraction.

The advantages are
that risks such as infection, inflammation, and bone loss can be managed in advance,
and that additional procedures such as bone grafting and sinus lifting can be performed more stably.
The drawback is
that the overall treatment period becomes longer because the healing time is longer.
Immediate placement (same-day implant)
Placement at the same time as extraction
This is a method in which the implant is placed right in the same location immediately after the tooth is removed.
It is commonly called a one-day implant.

The advantages are
that the treatment period is greatly shortened,
and that the number of implant surgeries and dental visits decreases, reducing the burden on the patient.
The drawback is
that it is not possible for every patient.
Next, let’s look more specifically at when immediate placement is possible.
Same-day implants
are possible in these cases
Implant surgery
First, the patient’s overall health should be in good condition, and there should not be severe inflammation in the oral cavity.
Some people think that if they have systemic conditions such as high blood pressure or diabetes,
same-day implants are completely impossible.
That is not the case.
Even if a patient has diabetes or high blood pressure, the procedure is possible if the values are well controlled and the overall condition is stable.
Also, the most important factor is the condition of the remaining bone.

The jawbone must be solid and remain sufficient
so that when the implant is placed immediately after extraction, it does not move and can achieve stability.

Implant surgery
So we determine whether same-day implant placement is possible using the following approach.
📌 Check the bone condition and whether there is inflammation through 3D CT analysis + a detailed intraoral examination
📌 For patients with underlying conditions such as diabetes or high blood pressure, check whether values are controlled through collaboration with the medical team
📌 Present a treatment plan based on the safest method with the highest chance of success for the patient
I’ll also show you an actual procedure case.

This was a patient who came in saying, "My lower right molar hurts."
The CT scan showed that the inner bone was in good condition, but the outer bone was very thin.
In this case, if same-day implant placement is attempted forcefully, the likelihood of bone resorption or placement failure is high.

So after extraction, I placed the implant immediately, but deeper toward the inner side.
Because the inner bone often experiences less bone resorption than the outer side and has a higher density,
I placed the implant deeper toward the inner side to secure better stability.
After implant placement, the bone also filled in well, and the prognosis looked good.

Then,
when do we choose delayed placement?

If inflammation is severe as shown above, or if there is substantial bone loss,
you should avoid immediate placement and first proceed with bone grafting to build up the bone sufficiently.

By performing bone grafting like this,
it is safer to proceed with implant surgery after the grafted bone has stabilized evenly.
The chance of success also becomes much higher.
Fast treatment is important, but
it is more important to first receive an accurate diagnosis of whether same-day implantation is the right method for you.
