Doctor, is it possible to correct a protruding mouth without extracting teeth?

Over the past 18+ years, I have treated more than 4,000 orthodontic cases,
and I have especially seen many patients who came in wanting to improve a protruding mouth.
Each patient had a different story, but they all came in with concerns like these:
"My side profile looks bad because only my mouth sticks out, even though the line beside my nose looks fine."
"My front teeth stick out, so my gums show a lot whenever I smile."


But when it comes to whether extraction is needed,
many people end up relying on online reviews or advice from people around them.
Some even decide based only on reviews of non-extraction orthodontics.
In my experience treating orthodontic cases for more than 18 years, not every protruding mouth requires extraction.
But on the other hand, not every protruding mouth can be solved with non-extraction orthodontics either.

The treatment plan changes depending on the type of protruding mouth
A protruding mouth can be broadly divided into 'dental protrusion' and 'bimaxillary protrusion.'
Dental protrusion refers to a case where only the teeth stick out forward.

If the teeth are tilted forward or have been pushed forward due to a lack of space,
after moving only the teeth backward, the lip line may improve sufficiently.
In such cases, good results can be expected with non-extraction orthodontics alone.
But the problem is bimaxillary protrusion, meaning the upper and lower jawbones themselves are generally protruded forward.

Because the jaw position itself is protruded,
even if the teeth are pushed back a little,
it is difficult to expect dramatic changes such as the lip line moving back significantly or the side profile becoming more attractive.
In such cases, surgical treatment may need to be considered.
However, in most cases, surgery feels burdensome and people want to avoid it..
and many want their impression to soften even a little through orthodontics alone.
So we explain what improvements are possible with orthodontics alone, without surgery,
through an overall analysis of the teeth, jaw, and lip line.
In some bimaxillary protrusion cases, if the dental component is more significant than the jawbone issue, meaningful change may be possible without surgery.
Is non-extraction orthodontics suitable for everyone?
A protruding mouth often means the upper front teeth are generally sticking out.
To move these front teeth backward, space is needed.
If there is enough space, then non-extraction orthodontics may be possible.
For example,
there are gaps between the front teeth
there are no premolars behind the canines
the teeth are generally small
If these conditions are met, moving the front teeth back without extraction is not difficult.
But in reality, most patients with protruding mouths visit with insufficient space.


In such cases, if you force the teeth backward, the teeth may tip over and the bite may become unstable,
and as a result, the teeth may protrude more and make the mouth look even more prominent.
Also, in some cases, one side may be treated with non-extraction and the other with extraction, such as non-extraction in the upper jaw and extraction in the lower jaw.
If this is approached without accurate bite analysis and simulation, malocclusion can develop or retreatment may be needed.

If you are considering orthodontic treatment, do not decide from consultation alone whether you truly need extraction or whether non-extraction is possible.
It is essential to make a diagnosis that also looks at the overall facial proportions and tooth alignment through CT scans and dental cast analysis.
Reviews saying that people benefited from non-extraction orthodontics do not mean the same result will happen for everyone.
