When drawing witches in cartoons,
you can see that the chin tip is made pointed and projected forward.
For example, this includes Maleficent’s queen and the Evil Queen, Snow White’s stepmother.

Among American female golfers, there is a player named Lexi Thompson.

She is a player with outstanding skills and looks.
But every time I see her on TV, I can’t help thinking that it would be nice if her chin were just a little less prominent
as a plastic surgeonㅠ.ㅠ
When the chin tip is pointed and projected forward, it can look stern and even make someone appear older.
The ratio that looks ideal to the eye varies by a very small amount.
Shall we look at a case?
This patient visited because they were worried that their chin looked pointed and projected forward in photos.

There is no history of fillers or fat grafting.
The impression of the chin is similar to Lexi Thompson’s.
If the chin were moved back a little more than it is now, the profile would come closer to the ideal line
and the face would change to a softer image.
Then, before going into surgery, shall we look at which profile is the most ideal?
One of the lines often mentioned when analyzing a person’s side profile is
Ricketts' E-line. Here, E stands for esthetic.

Based on Westerners, this explains how far the upper and lower lips should be from an imaginary line connecting the tip of the nose and the chin
for it to be ideal.
As shown in the photo, when the upper lip is 4 mm away and the lower lip is 2 mm away,
it is considered the most esthetic.
However, there is also a paper saying that in Koreans it is better for Ricketts' E-line to touch the lips,
so I attached it below.

I also agree with this and think that, rather than 4 mm and 2 mm, it is good for it to be slightly farther apart by a smaller amount.
That is how I explain it during consultations as well.
I drew Ricketts' E-line on the first photo.

Because the chin tip protrudes, the distance from the lower lip becomes greater than that from the upper lip.
In other words, the chin tip needs to move backward to be ideal.
Chin setback surgery was performed.

Left) Before chin setback surgery / Right) After surgery
As expected, I think it was the right thing to do!
You want to see Ricketts' E-line drawn, right?
I prepared it below.

Left) Before chin setback surgery / Right) After surgery
After surgery, the upper and lower lips became closer to Ricketts' line,
and the lower lip, which was farther away before surgery, can be seen to have become closer to the line.
For those who are curious about the CT scan, I prepared that as well.

Left) CT before chin setback surgery / Right) CT after surgery
On CT, the bone that had been projected backward is resorbed and becomes smoother.
If more setback is needed, a T-shaped osteotomy is used.
We looked at chin setback surgery to create an ideal side profile.
