Many of the people who visit the clinic say, “At some point, my eyes suddenly started to look older.”
If you look closely, the problem is often that sagging in the upper eyelids and changes in the lower eyelids are happening at the same time.
In cases like this, rather than thinking about each one separately, it is more natural to look at the upper and lower parts of the eyes as one structure and approach them that way.
Aging signs first noticed in the eyes before surgery
When the patient first came in, the most noticeable points were aging changes appearing in both the upper and lower eyelids at the same time.
The upper eyelids had skin sagging downward and covering the eye shape, creating a heavy impression every time the eyes were opened.
The lower eyelids had either bulging fat protruding outward or areas that looked sunken, giving an overall tired appearance.
With these changes overlapping, the eyes looked more fatigued than the person’s actual age.
In cases like this, it is important to look at the upper and lower eyelids together as one structure rather than addressing them separately.
That is why I explain that upper and lower eyelid surgery, considering both the upper blepharoplasty and lower blepharoplasty at the same time, can be a wiser choice in these cases.
Changes begin to appear right after surgery
Right after surgery, swelling and tightness are still present.
At this stage, what matters is whether the eye height looks natural, whether the left and right sides are balanced without strain, and whether there is any feeling of overcorrection.
For the upper eyelids, the plan was designed so that the structure used to open the eyes would function comfortably while removing loose skin.
For the lower eyelids, the focus was on removing unnecessary fat while adjusting the overall volume and balance so the under-eye area would not look hollow.
Four days after surgery, differences are visible during recovery
Around four days after surgery, just before and after stitch removal, the expression begins to soften little by little.
Swelling is still present, but the way the eyes are opened in daily life becomes more natural.
If you look at photos from this stage, you can see that the eyes open comfortably without appearing overly pulled, and the upper and lower eyelids connect naturally without looking awkward.
For upper and lower eyelid surgery, recovery must be smooth for the results to remain stable.
One month after surgery, balance is created around the eyes
After about a month, most of the major swelling around the eyes has gone down.
The upper eyelids are tidied so they do not block the field of vision, and opening the eyes requires less effort, making them feel much more comfortable.
The lower eyelids become less shadowed, and the overall impression looks brighter and cleaner.
At this point, many people say that their eyes look clearer without feeling artificial at all.
That is the standard I value most in upper and lower eyelid surgery.

When the upper and lower eyelids are viewed as a whole, they are structures that affect one another within the overall face.
In cases where the upper eyelids are sunken and the lower eyelids are bulging,
balancing them with a single upper and lower eyelid surgery can be an effective solution.
