When people think of improving breast sagging, the surgery that most people first think of and choose is breast augmentation surgery.

This is partly because augmentation is a relatively simple procedure, and because the breasts have sagged like air escaping from a balloon, some people think that if an implant is inserted into the sagging breast, like putting air back into a balloon, the breasts can return to being firm and lifted as they were before.

If you want to improve breast sagging by inserting an implant like this, when the nipple is positioned above the inframammary fold, the sagging can be improved to some extent by placing the implant.
In such cases, when an implant is inserted, the skin between the nipple and the inframammary fold stretches, making the breasts appear lifted overall and giving the result of skin that seems more elastic.

On the other hand, if the nipple has dropped below the inframammary fold, the breast cannot be changed into a lifted shape by inserting an implant.
In such cases, simply inserting an implant will make the area above the nipple appear fuller, but the sagging in the lower portion will remain, so it may actually look even more drooped.

That is why a breast augmentation mastopexy is performed in such cases.

With a mastopexy, the skin and tissue must be removed, while the lost volume is filled with an implant.
During the mastopexy, the remaining tissue may also be lifted and fixed with sutures to the upper pectoral fascia, but in such cases the effect can only be temporary.
Ultimately, no method can overcome time and gravity.

No matter how firmly the upper area is fixed, in the long term the heavy breast tissue cannot be held in place with sutures.
