Hello.
This is Mams Surgery and Plastic Surgery.
Even when surgery has been completed successfully,
some people continue to be concerned about the scars that remain afterward.
In particular, as time passes,
scars may become increasingly raised
or change by becoming darker.
At first, they may look like simple marks,
but as their shape gradually changes,
they may become an area that causes concern.
The scar-management process
is just as important as the surgical result.

Differences Between Surgical Scars and General Scars
When people think of scar treatment,
they often think first of dermatological procedures.
However, the scars commonly known to people
are mostly atrophic scars
that are indented, like acne scars.

In contrast, scars that occur after surgery
include forms with a different structure,
such as:
• Hypertrophic scars, in which the tissue becomes thickened
• Keloids, which grow excessively
In other words,
the approach itself needs to be completely different.

Differences in Treatment Based on Skin Thickness
The face and body
differ in skin thickness and structure.
In particular, areas such as the breasts and body
have a thicker dermal layer
and tend to show stronger reactions.

As a result,
laser intensity settings
and treatment methods inevitably vary.
Even for the same scar treatment,
"the approach must vary depending on the area."
Taking these differences into account
is key to determining the treatment direction.

Stages of Postoperative Scar Management
Scar management
proceeds according to a set sequence.
Immediately after surgery,
wound stabilization and recovery take priority.
As time passes, the process generally continues as follows:
• Dressing change approximately 3 days later
• Suture removal and taping approximately 10 days later
• Start of laser treatment approximately 3 weeks later
The timing of early management
may affect the overall result.

Important Choices in Early Treatment
At around three weeks after surgery,
tissue recovery
has not yet been completed.
At this stage,
rather than using a strong resurfacing laser,
it is important to focus on reducing inflammation
and supporting recovery.

Typical options include:
• Lasers that regulate vascular responses
• Treatments that promote early regeneration
In other words,
this is a stage aimed at encouraging stable recovery,
rather than aggressively removing tissue.

Why the Interval Between Laser Treatments Matters
Scar treatment
is not a procedure that becomes more effective simply because it is repeated quickly.
Scars on the body, in particular, have the following characteristics:
• A thick dermal layer
• A need for higher energy
• Increased reactions to stimulation
For these reasons,
treatment is generally recommended
at four-week intervals.
Shortening the intervals excessively
may instead lead to side effects.

Treatment Direction According to Scar Type
The approach to scars
varies depending on their condition.
For hypertrophic scars,
treatment with an Er:YAG laser
is used to:
• Precisely reduce scar tissue
• Minimize damage to the surrounding tissue

For atrophic scars, on the other hand,
long-pulse lasers are used,
with treatment focused on:
• Promoting dermal regeneration
• Improving skin density
In other words,
the treatment plan varies according to the scar's form,
even when the scars are otherwise similar.

Why Changes in Color Must Also Be Considered
Once a scar has stabilized to some degree,
treatment for color changes follows.
A scar that remains reddish
requires a different approach
from one that has become dark.
• Red scars → Vascular-focused treatment
• Dark pigmentation → Pigment-reducing treatment
This is how treatment proceeds in stages.
"Scars must be managed for both their form and color."

Treatment Duration and Realistic Expectations
Scar treatment
is not a process that ends in a short period.
Ongoing management may be needed
for several months at the shortest,
or for several years in some cases.
The important point is that
scars do not simply disappear;
rather, they gradually improve over time.
Instead of expecting
rapid results,
it is important to understand
the changes as a gradual, step-by-step process.
Thank you.
