Fat grafting also includes a process called "tunneling," which ensures close contact with the surrounding abundant blood vessels so nutrients can be supplied continuously.



♥ Tunneling means using micro-needles just beneath the skin to create space where the fat will be placed. Because space is created as if making a tunnel and the fat is then injected, the fat does not sag in the direction of gravity or become lumpy even after it has all been injected.



Three styles of chin microfat grafting

Chin fat grafting is designed according to the size of the chin and the shape desired.

In the case of a curved and asymmetric chin, the procedure is performed with the goal of smoothing the curve and achieving symmetry.

If the chin is short, there are methods that increase the length vertically and cases where protrusion is created in the front-lower direction at a 45-degree angle.

The design varies depending on what shape the person receiving the procedure wants, and fat grafting is performed to match the desired shape.

Effects gained from chin microfat grafting

By correcting a weak chin, concave chin, or asymmetric chin, the overall balance of the face is improved and the contour is restored to a normal shape.

Also, even after a chin implant is inserted and later removed due to side effects, the shape of the chin can be restored again with fat grafting.



Problems with implants for chin plastic surgery

For widening the jawline, solid implants such as silicone or Gore-Tex were widely used, but they had the burden of requiring an intraoral incision for insertion, and there was the added burden of carrying a foreign material in the chin. In some cases, side effects such as a foreign-body sensation or a tightening feeling occurred.

In addition, inflammation can occur after implant insertion, and if inflammation develops, the implant must be removed.

However, after implant removal, many cases are left with sequelae such as severe curved deformity of the chin.

Microfat grafting procedure for chin correction

1. Fat is harvested from areas where your own fat is highly accumulated, such as the lower abdomen, waist, or thighs.

• Because local anesthesia (painless sleep anesthesia) is used, there is no pain at all.

• A thin liposuction cannula is used to collect the fat into a syringe.



2. The harvested fat is placed in a centrifuge to remove impurities (local anesthetic, serum, red blood cells, etc.).

The purpose of centrifugation is to remove impurities contained in the fat, increase the concentration of the fat, and improve the fat's survival rate.



3. After centrifugation, water and oil are removed through a second concentration process.

To achieve a fat survival rate of at least about 50%, the oil and water must be removed through a second purification step after centrifugation.

For the transplanted fat to have a high survival rate, it must be free of impurities.

In practice, a single centrifugation step is often not enough to sufficiently concentrate the fat.

By removing oil and water, concentrated fat that is almost 100% pure can be obtained.

The purified fat is measured in 1 cc syringes as needed.

For lip fat grafting, about 2 cc of concentrated fat is grafted into each of the upper and lower lips.

For the chin (lower jaw area), about 5 cc to 7 cc of concentrated fat is grafted.



4. The fat is grafted into the area to be treated under local anesthesia.

The clinic uses micro-needles (micro grafting cannulas) developed in-house.

The micro-needle is a special grafting cannula used only at the clinic, capable of ultra-precise grafting.

In fat grafting, after the tunneling process is completed, space is secured for the fat to be transplanted before grafting is performed.

The principle of fat grafting is to transplant the fat evenly and uniformly across the target area so that it does not clump, while maximizing the survival rate as much as possible.

To do this, a small amount of concentrated fat must be finely grafted per unit area and spread evenly across multiple areas.

If a large amount of fat is grafted into one area, fat that does not receive blood supply will eventually necrose and be absorbed.

If the chin is small or has a sunken area, its volume can be increased with fat grafting.

There are almost no problems such as a foreign-body sensation or inflammation, unlike implants such as silicone or Gore-Tex.
Because the procedure does not involve cutting the skin, there is no need to worry about scarring.
Since it uses your own tissue, there is no need to worry about side effects.
Depending on the shape of the jawline, a variety of delicate forms of plastic correction are possible.

Precautions when correcting with microfat grafting

Fat grafting requires skilled technique and experience.The results can vary greatly depending on the level of skill, so careless fat grafting can actually make the face look worse, and caution is needed.
After the procedure, do not press on or impact the area until the fat has settled.
Running exercise can negatively affect the fat survival rate, so running should be avoided for about one month. Avoid drinking and smoking for about one week. Except in cases where fat was harvested from the legs, walking, weight training, and stretching may be resumed after one week.
You should not open or close your mouth too widely so that fat absorption is reduced.

Number of chin microfat grafting sessions

Chin fat grafting mainly involves transplanting fat evenly into the lower part of the chin muscles, inside the muscles, and into the fat layer.

In other words, three-dimensional fat grafting is performed.

In the chin, the fat survival rate is relatively high.

The survival rate is about 50% to 70%, and it is an area where fat cell survival is more stable than in other areas.

To build up the chin with fat, there are variables such as skin thickness and tissue flexibility, but in most cases, sufficient jawline contour correction can be achieved as desired.

Usually, one session is enough, and in some cases, two sessions can produce satisfactory results.

Long-term effects of fat grafting

Some people say fat grafting is ineffective because much of the fat is absorbed, but this is the case when the procedure is performed at a clinic with somewhat limited experience.

If fat grafting is performed accurately according to the principles, satisfactory results can be obtained, and once the transplanted fat survives, it is no longer absorbed and remains your own fat tissue for life, making it one of the safest and most effective procedures.

On average, one session, or in rare cases two sessions, of fat grafting can provide permanent, satisfactory facial contour correction.

Side effects of fat grafting

Fat grafting is one of the procedures with the fewest side effects.

However, inexperienced treatment providers may produce unsatisfactory results or side effects.

From fat harvesting to purification and transplantation, the entire process must be carried out very delicately, carefully, and in principle to achieve good results.

It is best not to receive the procedure from someone with insufficient experience or from a non-specialist without a plastic surgery license.

How to address fat grafting side effects

When fat grafting is performed contrary to principle due to a lack of experience, or when too much fat is transplanted because the amount was not properly controlled, the grafted area may become severely swollen, or even after the swelling goes down, the treated area may remain too protruded and look awkward.

This usually happens when a large amount of fat containing many impurities that was not properly purified is forced in.

In most cases it settles over time, but if the swelling is too severe, hyaluronidase to reduce swelling may be injected into the treated area to bring the swelling down faster. If too much fat was injected, a microcannula may be inserted to suction out and remove the transplanted fat again.

Over-injected fat can be more easily removed if it is suctioned out again within three months.

In some cases, transplanted fat becomes necrotic, calcifies, hardens, and must also be removed by suction.

However, if fat grafting is performed properly, these problems rarely occur.

Inflammation	Occurs when bacteria enter during the procedure, or when fat does not survive during the grafting process and dissolves

Pigmentation	When bruising or swelling is severe, iron components left in the bruise may deposit in the skin and oxidize, turning brown depending on the person's constitution; this sometimes occurs after under-eye procedures depending on the individual's constitution
Asymmetry	When the survival rate differs between left and right sides, or when the amount is not properly adjusted during the first procedure, causing left-right imbalance
High absorption rate	When enough fat does not survive to create the desired contour, and a large amount is absorbed, so the appearance before and after the procedure does not change much
Awkward shape	When fat survives excessively or is transplanted to the wrong area rather than the proper one, disrupting the overall balance of the face

Methods for fat grafting without side effects

1. Perform the procedure in a sterile environment

As with all surgeries, fat grafting must be performed with completely sterilized instruments and materials.

After the procedure, the inside of the suction cannula and grafting cannula should be thoroughly cleaned with a brush so that no residual fat remains, and then disinfected with a high-pressure steam sterilizer. Anesthetic agents, saline, and similar materials must also be carefully managed so they are not contaminated.

Thorough disinfection and proper material management can prevent bacterial infection and inflammation.



2. Multi-step purification of fat

Fat purification must remove impurities completely through several stages to maximize the effect.

Harvested fat usually contains about 80% impurities, and only about 20% is actually usable.

If the remaining impurities (blood, serum, anesthetic, oil, water, etc.) are not removed, the results will not be good no matter how well the grafting is done.



3. Pre-treatment space preparation before fat grafting

A high survival rate should make it possible to achieve satisfactory results with just one procedure whenever possible.

The fat must be planted evenly across multiple layers, and before that, space must be secured in the area where the fat will be transplanted so that the previously grafted fat is not compressed by surrounding tissue, allowing it to survive well.



4. Precise and even grafting across multiple tissue layers

The key to fat grafting is expressing a soft and natural contour.

For a soft contour, it is best to graft in a three-dimensional manner, evenly across multiple layers.

The grafting locations include the subcutaneous layer, muscle layer, and fascia layer. The fat must be spread through the tissue as if scattering seeds evenly to achieve a smooth contour without unevenness.
