Chin Microfat Grafting

Chin fat grafting

is aimed at restoring a naturally balanced chin

for microgenia, a small chin,

a recessed chin,

or asymmetry in the chin line.

Indications for chin microfat grafting

○ When the chin is small

○ A chin that has developed a contoured deformity after implant removal

○ A concave chin with a sunken center

○ A chin with scars and contour irregularities due to trauma

○ An asymmetric chin with contour irregularities

○ A recessed microgenia

Three styles of chin microfat grafting

Chin fat grafting is designed with consideration for the size of the chin and the desired shape.

In the case of a contoured and asymmetric chin, the procedure is performed with the goal of smoothing the irregularities and improving symmetry.

If the chin is short, there are cases where the vertical length is increased, or where projection is created toward the front and downward at a 45-degree angle.

The design varies depending on the shape the patient wants,

and fat grafting is performed to match the desired shape.

Effects of chin microfat grafting

By correcting a microgenia, recessed chin, or asymmetric chin, the overall balance of the face is improved

and the chin is restored to a normal contour.

In addition, even after an implant has been inserted into the chin and then removed because of side effects,

fat grafting can be used to restore the chin shape again.

Problems with implants for chin plastic surgery

For chin line augmentation, solid implants such as silicone or Gore-Tex were widely used.

However, they required an incision inside the mouth for insertion, and there was also the burden of having a foreign material placed in the chin.

In some cases, side effects such as a foreign-body sensation and a feeling of tightness may occur.

In addition, inflammation can occur after implant insertion, and if inflammation develops, the implant must be removed.

However, after implant removal, sequelae such as severe contour deformity of the chin often occur.

Therefore, if deformity occurs in the chin after implant removal, correction should be performed with fat grafting.

Microfat grafting procedure for chin correction

1. Fat is harvested from areas where a large amount of fat has accumulated, such as the lower abdomen, waist, or thighs.

• Because local anesthesia (painless sedation anesthesia) is used, there is no pain at all.

• A fine liposuction cannula is used to collect fat into a syringe.

2. The harvested fat is placed in a centrifuge to remove the local anesthetic and serum, among other components.

The purpose of centrifugation is to remove impurities, reduce side effects after grafting, and increase the survival rate of the fat.

3. After centrifugation, the fat undergoes a secondary concentration process to remove moisture and oil.

To achieve a fat survival rate of at least about 50%, the fat must undergo a two-step purification process after centrifugation to remove oil and moisture.

For the grafted fat to be free of impurities, the survival rate is higher and the results are more effective.

In practice, a single centrifugation step is often not enough to sufficiently concentrate the fat.

By removing oil and moisture, it is possible to obtain nearly 100% concentrated fat.

The purified fat is measured into 1 cc syringes in the required amount.

For the chin (mandible), about 5 to 7 cc of concentrated fat is grafted.

4. The fat is grafted into the area to be treated, which has already been locally anesthetized.

At our clinic, we use our own micro-needle (micro-grafting cannula) developed in-house.

The micro-needle is a special grafting cannula used only at our clinic that allows ultra-precise grafting.

In fat grafting, after the tunneling process is completed, space is secured for the fat to be grafted, and then the grafting is performed.

The principle of fat grafting is to distribute the fat evenly throughout the area to be grafted so that it does not clump, while maximizing the survival rate as much as possible.

To do this, a small amount of concentrated fat must be finely grafted per unit area and distributed evenly across multiple sites.

If a large amount of fat is grafted into one area, the fat that does not receive blood supply will eventually necrose and be absorbed.

Effects of the fat grafting tunneling technique

Rather than injecting a large amount into one area with the tunneling technique, we perform three-dimensional microfat grafting with 100% concentrated fat, distributed evenly in multiple layers.

Customized design is possible using a delicate and careful injection method.

Fat grafting also includes a process called "tunneling," which allows the fat to be in close contact with the rich surrounding blood vessels so that it can continuously receive nutrients.

Tunneling means using a micro-needle just under the skin to create space where the fat will be placed.

Because the space is created as if making a tunnel and the fat is then injected afterward, even after all of the fat has been placed,

it does not sag in the direction of gravity or become uneven.

Examples of chin (mandible) fat grafting procedures

If the chin is small or has a sunken area, fat grafting can increase the chin volume.

For chin line augmentation, solid implant materials such as silicone or Gore-Tex were widely used in the past.

However, there was the burden of needing to make an incision inside the mouth for insertion, and there was also the burden of carrying a foreign material in the chin.

In some cases, side effects such as a foreign-body sensation and a feeling of tightness may occur.

For those who are afraid of implanting foreign material because of these issues, natural chin line correction without side effects is possible through fat grafting.

The effect of chin fat grafting is comparable to implants or mandibular advancement surgery.

Because most chin shapes that can be corrected with implants or jawbone surgery can also be created with fat grafting, most microgenia and recessed chin cases can be naturally corrected with fat grafting without the need for implant insertion or jawbone surgery.

Advantages of chin line correction with fat grafting

○ The procedure is simple.

○ There are almost no problems such as foreign-body sensation or inflammation, unlike implants such as silicone or Gore-Tex.

○ Because the procedure does not require an incision in the skin, there is no need to worry about scars.

○ Because it uses autologous tissue, it is soft and looks natural.

○ Depending on the shape of the chin line, delicate shaping in various forms is possible.

○ Since the grafted fat remains permanently as your own tissue, the effect can be maintained for a long time.

Precautions when correcting with fat grafting

○ After the procedure, do not apply pressure or impact to the area until the fat has settled.

○ Running exercise should be avoided for about one month because shaking can reduce the survival rate of the fat.

○ Avoid alcohol and smoking for about one week.

○ Do not open or purse the mouth too widely so that the fat is less likely to be absorbed.

Number of chin fat grafting sessions

Chin fat grafting is mainly performed by evenly grafting fat into the lower part of the chin muscle, within the muscle, and in the fat layer.

In other words, three-dimensional fat grafting is performed.

In the case of the chin, the fat survival rate is relatively high.

It shows a survival rate of about 50-70%, and compared with other areas, this is a site where the grafting of fat cells is more stable.

There are variables such as skin thickness and tissue flexibility when augmenting the chin with fat,

but in most cases, sufficient chin line contour correction can be achieved as desired.

In many cases, the procedure is completed in a single session,

and in some cases, satisfying results can be achieved with about two sessions.

Long-lasting effects of fat grafting

Some people say fat grafting is ineffective because much of the fat is absorbed,

but this applies to cases where fat grafting was performed poorly at a clinic without experience.

If fat grafting is performed accurately and according to principle, satisfying results can be achieved,

and once the grafted fat survives,

it is no longer absorbed and remains as your own fat tissue for life, making it one of the safest and most effective procedures.

On average, one to as many as two fat grafting sessions can provide permanently satisfying facial contour correction results.

Side effects of fat grafting

Fat grafting is one of the procedures with the fewest side effects.

However, side effects can sometimes occur because of inexperienced technique by the practitioner.

Severe swelling due to excessive fat injection and calcification caused by fat necrosis

Uneven contours, reduced procedural effect due to excessive fat absorption, and redness due to inflammation may occur,

but these are issues related to the practitioner’s experience and skill.

If the procedure is performed correctly and according to principle, these problems rarely occur,

so it is advisable not to undergo the procedure at a clinic with no fat grafting experience or by a non-board-certified plastic surgeon.
