Ulthera Prime is the new model of Ulthera.
While the lifting effect is important, one of the things many people ask about in the clinic is pain.
Looking at reviews, some people say, “It didn’t hurt much,”
while others say, “It really hurt so much that I teared up.”
If you read many different reviews, it can actually be even more confusing.
In fact, pain varies depending on the device model used, the treatment area, the practitioner’s settings, and each person’s pain sensitivity, so it is difficult to say objectively that it is definitely one way or another.
Still, let’s talk about which aspects feel less painful than Ulthera, which areas may be especially painful during treatment, and finally how pain can be reduced.

Ulthera Prime
Why does pain occur?
Ulthera Prime is a lifting device that uses high-intensity focused ultrasound (HIFU) to create precise thermal coagulation points deep in the skin, especially in the SMAS layer (the fascial layer).
Because this heat stimulates tissue, contracts collagen, and induces regeneration, the energy emitted by the device itself inevitably causes pain.
Ulthera Prime pain
Is it less painful than Ulthera?
However, compared with the existing Ulthera, Ulthera Prime has significantly reduced pain.
The original Ulthera concentrated strong energy at a single point, whereas Prime was designed to disperse it more finely so that it “reduces stimulation while maintaining effectiveness.”
In addition, the new cartridge disperses contact pressure with the skin, minimizing thermal damage and reducing pain.

And because treatment can be performed faster than with the previous Ulthera, the amount of time you feel pain is reduced as well.
When we asked our managers and people who had previously received Ulthera, they all said Ulthera Prime was definitely less painful.
However, because “less painful” can mean different things to different people, it is not completely painless, and the level of pain can vary depending on the treatment area and individual differences.
Ulthera Prime pain
Why does it differ by area?
Ulthera Prime pain also differs by treatment area. Let’s go over each area.
- Temples (temporal area)
The temples have thin soft tissue between the bone and skin, and because nerves are close by, pain sensitivity is high.
This is an area that many reviews describe as the most painful during Ulthera treatment.
- Below the jawline (lower jaw, double chin area)
There is little soft tissue under the skin, and vibrations can be transmitted near the gums or jawbone, making it easy to feel dull pain or a “tingling” sensation.

- Cheeks and area near the cheekbones
Because this area relatively has more fat, it tends to be less painful.
However, if the energy is delivered strongly, it can contact internal nerves and feel like “facial pain.”
- Around the eyes, under the eyebrows
The thin skin layer and the concentration of blood vessels and nerves make this area highly sensitive to stimulation.
The closer it gets to the eyes, the more psychological tension can also amplify pain.
As shown above, even on the same face, the thickness of the tissue and the location of nerves differ by area.
That is why some areas are more sensitive to pain, while others are less sensitive.

How can pain be reduced?
To reduce pain, you can apply anesthetic cream or use sleep anesthesia.
In addition, pain can also be reduced through the treatment itself.

Ulthera Prime is not a device that delivers a fixed amount of energy uniformly; the practitioner can adjust the intensity, depth, and number of shots.
So for areas sensitive to pain, adjusting the intensity and widening the spacing between shots in nerve-prone areas can help reduce stimulation and, in turn, pain.
In this way, treatment can be tailored to the patient’s pain sensitivity, and for that reason, pre-treatment consultation is important.
Ulthera Prime is a device that provides a clear lifting effect, but the associated pain cannot be ignored.
However, what matters is understanding why certain areas hurt and designing the treatment with a strategy suited to that, which can make it much less painful than expected and lead to more effective results.
If you have been putting off treatment because you are worried about pain, please look for a doctor who understands your concerns through consultation.
