There are various criteria for extraction, but
in most cases, if a tooth has already undergone root canal treatment, you can think of it as having been treated as much as possible.

Today’s patient also had a problem with a previously treated tooth,
so we proceeded with extraction!

Reason for visit: My gums keep swelling on both sides

This is a man in his 50s who visited Hanam City Hall Dental Clinic.
He said that his gums kept swelling from time to time.
After examining with an X-ray,
the gum bone had generally receded.
In particular, the molar areas on both sides of the upper teeth had receded a lot,

There is pus around the root,
so it looks darker than the surrounding area because a lot of the gum bone has dissolved.

the patient asked if there was any way to save it.
When a tooth that has had root canal treatment develops a problem,
there are only a few options.
Especially if it is a molar?
And the gums are not in good condition?
It has to be extracted.
(If it is a front tooth, there is at least a chance to try to save it.)
Even on the CT scan, the bone had dissolved too much,
so we recommended extraction.
If we left it longer, the swelling would keep recurring,
and the gum bone would continue to dissolve.

Since it was an upper molar with weak bone...
we performed the first bone graft at the same time as extraction.
Even when bone is grafted at this time, only about 8 out of 10 remains,
and the rest is absorbed.
For implant surgery,
we filled the bone as fully as possible
and checked again after 4 months.

However, in order to proceed with implant surgery,
it is safest to wait 6 months.
The quality of the bone in the upper jaw is weak,
with a sparse, scattered structure~
If we force it too early,
it increases the chance of implant reoperation
and may cause maxillary sinus inflammation
or other complications..

After the first bone graft following extraction,
we performed implant surgery after 6 months had passed.
We opened the gums and checked the condition,
and the bone quality was poor.
Even after waiting 6 months after the bone graft.
If treatment had been received earlier,
before the problem became more serious,
we could have shortened the treatment period...
With a bit of regret,
we placed the implant, added and grafted more bone where it was lacking,
and waited about 3 more months.

We used an oral scanner to make the prosthesis.
Since all the teeth in the full mouth are scanned,
it is possible to compare the left and right teeth,
and
the relationship of the upper molars can be captured more precisely than with rubber impression material~

there won’t be any treatments that take a long time.
After educating them that maintenance is important, we scheduled regular checkups.
Quick treatment is by no means the best treatment method.
If you want treatment to be done quickly and in a short period of time,
visit the hospital as soon as you notice a problem!
If you receive treatment early, there is not much to treat.
But if you feel symptoms and then visit later,
if you brush it off thinking it hurt and then got better...
recurrence and healing repeat...
so the condition gets worse ^^
