This was the first question from a patient who recently visited our clinic.
They said they had been told at another dental clinic that the root inflammation was so severe that the tooth had to be extracted.
But after not giving up and trying treatment,
they are now using it healthily.
Today, I’ll share the treatment process that allowed us to save a tooth with root inflammation without extracting it.

What happens if root inflammation in a tooth is left untreated?
Root inflammation can be compared to a tree root slowly rotting away.
Even if it looks fine on the outside,
just as a tree begins to wobble when the roots underground become diseased,
a tooth gradually becomes unstable when inflammation develops in the root.

more serious than expected.

The inflammation had spread deeply between the roots,
and the surrounding bone supporting the tooth had already been significantly lost.

In addition, more than half of the palate was severely swollen,
and pus was coming out of the gums.

In this condition, the looseness is very severe,
so most dental clinics would recommend extraction.

Honestly, when I first saw the patient,
the treatment seemed very difficult.
The looseness was severe, and the root inflammation had spread widely.
However, I judged that it would not be right to simply say, "We can save it."
I explained the exact situation to the patient as it was.
"The chance of saving it is about 20%."
I also explained thoroughly that if treatment failed, other treatment options such as extraction followed by an implant would ultimately need to be considered.
Even so, the patient said they wanted to try treatment until the very end.
They were so determined that they said they felt as if they were grabbing at even a single straw.
Their sincerity was felt by us as well,
so we decided to do our best if there was even a 20% chance.
The treatment process was carried out as follows.
First, we began by removing the inflammation at the root tip through root canal treatment.
Even when root canal treatment is done very carefully on a tooth with root inflammation,
it is very difficult to remove the inflammation completely.
In particular, inflammation in the area between the roots (the furcation area)
is difficult to access and very hard to remove.

But we did not give up and repeatedly performed the process of carefully removing and disinfecting the complex root canals.
After the inflammation had subsided to some extent,
we used a material called MTA to firmly seal the root tip.
What is MTA? It is a material with high biocompatibility,
and it is very effective in promoting healing of root-tip inflammation
and helping damaged roots recover.

Unfortunately, MTA treatment is not covered by national health insurance.
However, in actual cases of application,
it helps with healing root-tip inflammation,
and when used appropriately if necessary, it can improve treatment outcomes.

As the inflammation gradually subsided, the tooth began to stabilize.
The looseness also decreased noticeably.
In the end, we placed a prosthetic restoration (crown), and the patient is now using it healthily.

Many people feel despair when they are told that a tooth must be extracted.
But before giving up, I recommend checking once more.
To be honest,
we also cannot save every case.
But if there is even a small chance of saving it,
we simply try once more for that possibility.
However, for treatment to succeed,
the patient’s willingness is also very important.
No matter how much I want to save a tooth,
if the patient says, "Please pull it quickly and place an implant,"
there is nothing I can do.
Conversely, if the patient has a strong will to continue treatment until the end,
as in this case, then even with low odds, we can do our best
and challenge it together.
