The biggest cause of gum recession is
periodontal disease (gum disease).
Gums receded due to tartar
As tartar builds up in the space between the teeth and gums,
after which inflammation begins in the gums,
it gradually dissolves the underlying alveolar bone (jawbone),
and the gums recede along with it.
- Brushing too aggressively
Many patients say,
“I thought brushing harder would clean better.”
But when you press the toothbrush down firmly,
the bristles do not just clean the tooth surface;
instead, they can start wearing away the gums.
Cervical abrasion caused by a side-to-side brushing habit
With this habit, the gums around the root area gradually
pull back as if they are being pushed backward,
and you may feel that the gums are slowly receding.

If you clench or grind your teeth while sleeping,
or if you have abnormal bite problems,
excessive force is continually transmitted to the teeth and jawbone.
When this force is repeated, microscopic damage accumulates in the jawbone,
and at some point, the amount of bone begins to decrease.
Then the gums covering that bone also recede.

The reasons this patient came to our clinic were as follows.

Over time, these symptoms worsened and became a concern.
When we examined the mouth,
tartar was visible inside the gums in the affected area.

After taking X-rays and conducting examinations to confirm the exact condition,
we found that periodontal disease had progressed due to tartar,
and part of the jawbone had been lost.

When the jawbone decreases,
the support for the teeth weakens.
As a result, the teeth gradually move downward from their original position,
and they start to look longer.
In cases like this, extraction is not uncommon.
However, in this patient, the bone supporting the front teeth was not completely destroyed,
and the tooth mobility was not severe,
so we were able to plan a treatment approach that preserved the natural teeth.
In addition, the patient had a history of long-term treatment for colon cancer,
so she said she wanted to keep the teeth as much as possible.
With the same goal in mind, we proceeded with gum treatment and conservative care first,
instead of an aggressive extraction.
Our goal was to remove the tartar that had caused the bone loss and carefully remove the inflamed tissue,
then restore the gums to a healthy state.
Also, because the tooth root was exposed and worn down,
and there were sensitivity symptoms, we performed root canal treatment as well.

After that, to improve the spacing between the teeth and the chipped areas,
we adjusted the size distribution with the adjacent teeth
and finished with zirconia crowns.
The front teeth are the most visible area,
so even a slight difference in size or shape stands out quickly.

In cases with wide spaces like this patient’s,
simply filling the empty space tightly
can easily make the front teeth look too large.
So rather than focusing on how much to fill,
the key is how to distribute the teeth.
