"If I get whitening, will this turn white?"
In those cases,
unfortunately, there are quite a few times when I have to say,
"Whitening will likely be difficult."
Not all discoloration can be resolved with whitening.
Today, I’ll talk about the types of discoloration that are difficult to improve with whitening,
and a real case in which front tooth discoloration was improved.

General staining or yellowish discoloration
can often be improved with whitening.
But discoloration that starts inside the tooth
is very difficult to improve with whitening.
Types of discoloration that are difficult to improve with whitening
- Discoloration caused by antibiotics
If a person took antibiotics such as tetracycline
when they were young,
discoloration develops deep inside the tooth.

A characteristic feature is the appearance of horizontal brown or gray bands.
Because this discoloration comes not from the tooth surface
but from pigment deposited in the inner dentin,
it is difficult to improve with whitening agents.

If a tooth is traumatized or the nerve is damaged,
the tooth can turn dark gray or brown.
In these cases as well, since the discoloration starts inside the tooth,

These are cases where there are white or brown spots

In such cases, if whitening is done,
areas other than the spots become brighter,

So rather than whitening, prosthetic treatments such as veneers or crowns

A patient who visited us recently left a strong impression on me.
There was spacing between the front teeth,
and clear horizontal brown lines were visible, so every time they smiled,
they said they covered their mouth.

At another clinic, they were told that orthodontic treatment should be done first
to improve the spacing in the front teeth,
but because it would take a long time and the cost burden was large,
they had simply been putting up with it.
Then, as they kept worrying about the color and spacing of the front teeth,
they came to our clinic,

"If I get whitening, will it become white?"
Upon closer inspection, it was a typical case of antibiotic discoloration.
As I mentioned earlier, antibiotic discoloration
involves pigment deposited deep inside the tooth,
so it is very difficult to expect teeth whitening to work.

improvement would be difficult with whitening alone.
Instead, I explained that the most effective approach would be to resolve the spacing and discoloration at the same time
with veneers.
After much thought, the patient decided to proceed,
and we treated the four front teeth that are most visible when smiling
with veneers.

We carefully matched the size and proportions
so the spacing was filled naturally
without making the teeth look too large.
The color was made within the brightness range the patient wanted,
so it would look natural and healthy.

After completion, the brown horizontal lines were no longer visible,
and the tooth color was bright and uniform.

The spacing between the front teeth was also filled naturally,
and the patient said they felt more confident when smiling.
Above all, it was very rewarding for me as well
that a problem they had worried about for a long time
could be resolved all at once in a relatively short period.

Not all tooth discoloration can be resolved with whitening.
But even discoloration that cannot be resolved with whitening
can still be improved aesthetically enough through veneers, crowns, or other prosthetic treatments.
