In the course of treatment, there are times when we recommend completely different treatments to patients who visit us with similar concerns.
“I want to improve my front teeth because their color is dark and their shape is not attractive.”
Even when the goal is the same, for some patients we recommend laminates, and for others zirconia crowns.
Why is that?
Today, through the cases of two actual patients,
we will go step by step through why different treatments were chosen even though both were seeking aesthetic improvement of the front teeth. ^^

A laminate is a treatment that improves color and shape by applying a thin layer to the outer surface of the tooth.
It is similar to a false nail in that it “smooths the surface and attaches something to make it look better.”

Because laminate is a bonded treatment,
for it to adhere well and last a long time,
the outer layer of the tooth,
emal (enamel), needs to be healthy,
which is favorable for maintaining bond strength.
On the other hand, a crown is a treatment that covers and protects the entire tooth.
Since it wraps around the tooth like putting on a hat, it can be a more stable choice when the tooth is weakened
or when chipping happens repeatedly.

The choice between laminate and crown is mostly decided by the following three factors.
- Bite alignment and chewing force
If the chewing force is strong,
or if the force is concentrated on the front teeth, as in an edge-to-edge bite,

laminate is more likely to chip or come off due to its structure.
Conversely, if the bite is close to normal
and there is not excessive force on the front teeth,
laminate can often be used stably for a long time.
- Presence of wear and fracture
If the teeth are already significantly worn down
or the front teeth are chipped,
If the teeth are worn down and chipped
first you need to check the cause of why they are worn or chipped.
That way, you can determine whether the conditions are suitable for a bonded treatment like laminate,
or whether a treatment that covers and protects, like a crown, is needed.
- The extent of enamel and existing restorative material (resin)
For laminate, the outer layer of the tooth,
enamel, needs to be healthy for favorable bond strength.
If the tooth surface is healthy
and there are few large restorations,
it is more likely to last longer after bonding.
When existing resin is extensively present
On the other hand, if there is a wide area of existing resin
or if there are many cracks or areas of damage,
laminate may be less favorable in terms of retention.
Case 1) A patient treated with laminate

Let’s look at a case of a patient who visited us wanting to naturally improve the color and shape of the front teeth.
This patient was concerned because the front teeth were spaced apart and the tooth color was dark,

but when we examined the bite,
it was close to a normal bite,
and there was not a strong pattern of force being concentrated on the front teeth.
Also, overall there was no significant tooth wear or chipped areas,
and the amount of enamel on the tooth surface was sufficient.

In a case like this, proceeding with laminate does not carry a major disadvantage in retention,
and it can naturally improve discoloration and spacing between the front teeth with minimal tooth reduction.
Rather than broadly expanding the treatment area,
we proceeded with laminate only on the upper front 4 teeth, which draw the most attention.

Even without treating the front 6 teeth,
this was a case where the spacing and color concerns could be naturally resolved with just the front 4 teeth.
Case 2) A patient treated with zirconia crowns

This was also a patient who visited us wanting aesthetic improvement of the front teeth.

Although the concern looked similar on the surface,
there were clear differences in the diagnostic process.
The bite force was strong, and overall the teeth were significantly worn down.
The ends of the front teeth were also heavily chipped.

In this case,
if we chose a thin bonded treatment like laminate,
there would be a higher chance of chipping or coming off.
Our goal was to protect the teeth from breaking while also addressing the aesthetic aspect.
Because strength was needed in this situation, we chose zirconia,
and to align both the front tooth line and function,
we planned treatment for 4 upper teeth + 5 lower teeth.
This patient had a relatively strong bite force,
so we paid special attention to distributing the bite.
This is because if the front teeth come into excessive contact or force becomes concentrated there,
no matter how well a prosthetic is made, it can still break.

So after final placement,
we carefully checked the contact points and finished by ensuring the force would be distributed properly.
Even for the same front-teeth aesthetic improvement,
depending on the bite and the condition of the teeth,
the appropriate treatment may be laminate,
or crown may be the safer option.
“I heard laminate is good, so why are you recommending a different treatment for me?”
We often get this question.
In the end, treatment must be chosen based on the conditions of the teeth you have in order for it to last.
