Except in cases caused by trauma,
fractures due to cavities usually
progress over a long period of time.
In particular, older adults often have reduced sensation,
so even when cavities are deep, there is little pain,
and the condition may progress without them realizing it.
One day, the tooth suddenly breaks,
and that is often how it is discovered.
This patient was similar.
Today, through this patient’s case,
I will explain step by step
how a tooth breaks due to cavities,
and in what cases it can even lead to implants. ^^
First visit, when they came because a prosthesis had fallen off

"The implant prosthesis on my upper left molar
fell off."
That was the reason this patient first came to our clinic.

First, we checked the condition of the detached implant prosthesis
and reattached it properly,
but the problem was that there were cavities
in multiple areas.
After examining the mouth,
there was decay progressing between the teeth
in the #12 area (right front tooth).

The gums around the upper front teeth (#11~22) and bridge
had quite severe inflammation,
and it was accompanied by pus drainage.

At this point, it seemed unlikely that the teeth
would last for long.
The patient said,
"Today I only want to fix the detached implant,"
but
in cases like this,
even if we do not begin treatment right away,
I make it a point to tell patients about the problem areas.
Because passing by without knowing
and knowing in advance and setting a management plan
are very different.
At the very least,
if they remember, "Ah, they said this area was not good,"
then they become a little more careful
with brushing and maintenance,
and that small difference can ultimately
help extend the life of the teeth.
One year later, the patient returned because the front teeth and bridge had broken

About a year later,
the patient came back,
this time saying the front teeth had broken.
After checking, it was the front tooth area I had mentioned before.

The condition had progressed even more than it had a year earlier.
As cavities accumulated over a long period,
the tooth had effectively been reduced to only the root,
and the root had also undergone resorption,
becoming shorter.
In such cases, even if a post is placed
and a new prosthesis is made,
there is a high chance it will not last long.
So we decided to extract the three front teeth
and restore them with implants.

Fortunately, when we checked the CT,
the height and thickness of the bone were relatively good,
so we were able to place the implants
without bone grafting.
After the healing period,
we completed the final prosthesis
about three months later.

The implants we had placed were being used well,
but this time the upper right teeth next to the implants,
(#13, #14),
had broken, so the patient came in.

After checking, we found again that
the extent of decay was wide
and the roots had shortened,
so we judged that it would be difficult to save them conservatively.

extraction followed by implants,
and this also was completed with prosthetics about three months later.

In this patient’s case,

so over time, the weaker areas started to show problems

the nerves in the teeth had become thinner,
so even if cavities progressed deeply,
it was difficult to feel pain.
In particular, between teeth or inside prostheses,
problems are even more likely to be discovered late.
So it may look like the tooth suddenly broke,
but in reality, it is often the result
of long-term accumulation.

Especially for older adults,
sensation can be reduced,
so even if cavities are deep,
there may be little pain.

Around bridges and crowns,
even if they look fine on the outside,
they often progress from the inside.
- Detecting the problem before the tooth breaks is much better than treating it after it breaks
Once a tooth has broken, the options become limited,
and there is a high possibility it will lead to extraction and implants,
so getting regular checkups in advance and preventing problems
is the only way to avoid a bigger issue.
