This patient

had the upper four front teeth

After taking a close look inside the mouth,

the causes could be divided into three main points.

1. Teeth worn down from contact with each other

The upper front teeth, which had been made unusually long,

kept hitting the lower teeth,

wearing them down severely enough

2. An alignment that looked even more crooked

Because the lower teeth had worn down and become shorter,

the upper teeth looked relatively more uneven

and crooked.

3. Receded gums and pain

As the gums receded,

the roots became visible,

and the metal edge inside the prosthetics

showed through in a dark color.

Most of all, the patient complained of pain,

saying, "It throbs when I chew."

The detailed examination found that the previous root canal treatment had not been properly completed,

and inflammation had developed inside.

In the end, all of the existing materials had to be removed,

1. Removing the existing material itself is a challenge.

The old material that has hardened inside the root canal

must be carefully removed

without damaging the inner wall of the tooth.

It’s like working inside a narrow tunnel

without scratching the walls.

2. It is difficult to find the original location and direction of the root canal.

If the previous treatment was insufficient,

the canal may be blocked

or its direction may be twisted,

so finding the original path again is not easy at all.

3. The tooth is weakened and at risk of breaking.

A tooth that has already been treated once

is structurally weaker,

so it must be approached very carefully,

because it could split.

This patient also had a condition where the nerve had not been fully removed to the root tip,

but fortunately, we were able to find the hidden canal well

and complete the treatment safely after three thorough rounds of disinfection.

After finishing the root canal treatment,

the most important thing was the line of the front teeth when smiling.

However, there was a point of concern.

Because the lower teeth had already worn down a lot

and become shorter,

if the upper front teeth were aligned straight,

certain areas would not interlock with the lower teeth

and would sit slightly apart.

I explained this honestly to the patient.

Fortunately, the patient also agreed, saying,

"As long as I can chew well with my molars,

I really want beautiful front teeth this time."

So we proceeded with a design focused on aesthetics.

In addition, the patient’s chewing force (occlusal force) was very strong,

so ordinary porcelain carried a high risk of breaking.

which has far superior strength,

and did not compromise on durability.

1. Intraoral scanner

The old method involved taking an impression with rubber material

and then making a plaster model to fabricate the prosthetic.

it was difficult to capture

the natural translucency

and subtle color differences of real teeth.

As a result, when the prosthetic was placed,

it could look slightly out of place because its color differed from the surrounding teeth.

it records even the fine texture and translucency

2. In-house dental lab

This precise digital data is sent immediately to the dedicated in-house lab.

Because the technician directly checks not only the numbers in the data,

but also the patient’s actual smile and lip line,

more detailed work becomes possible.

Adjustments like

"make the tip just a little rounder"

or

"make the color slightly more translucent"

can be made right on site,

resulting in a customized prosthetic made only for that one patient,

not a mass-produced product.
