Hello.
I am the director of Seoul Bardi Dental Clinic.
"My teeth seem
to have gotten longer than before.
They wobble even when I touch them with my tongue..."
Since adult teeth
would not suddenly grow taller,
from the patient’s point of view,
it is truly startling.
Usually,
people often brush it off, thinking,
"I guess my gums have receded a bit with age."
But in fact, this is a surprisingly candid SOS signal
sent by the gums.
Because the jawbone that should firmly hold the teeth in place
is being resorbed,
it means the teeth are losing support
and being pushed upward.
If you ignore this signal,
a situation may come later
that is truly difficult to treat.
Today, I’ll explain the process of resolving
protruding and wobbly teeth in a patient in their 70s
through a case study.^^
The reason the teeth rose upward and became loose over 2 years

Two years earlier, the bone condition had been good
and there was no looseness,
but when we met the patient again two years later,
the two lower front teeth had noticeably protruded upward
and were severely loose.

The cause was an inflammation at the root,
which had dissolved the jawbone
and removed the support.
Like a tree swaying after the soil around it has washed away,
with no bone left, the teeth had been pushed upward.
If left untreated,
the teeth will eventually fall out on their own.

Rather than extraction,
we first consider ways to save the teeth.
Why the Signal Sent by Protruding Front Teeth and Wobbly Teeth Should Not Be Ignored
As someone who is truly committed to "preserving natural teeth,"
I had a lot to think about when I saw this patient’s diagnosis.
The two middle teeth had no choice but to be extracted,
but the teeth on either side were still holding their positions quite well.

However, I was concerned about the overall reduction in the jawbone around the gums,
so I explained two options in detail to the patient.
230602
A "bridge," which can preserve as many natural teeth as possible
but may place a burden on the remaining teeth,
and an "implant," which cleans up even the adjacent teeth
and fundamentally addresses the cause of the inflammation,
though it is unfortunate.
The patient thought for a moment and then said,
"Rather than going through trouble again later,
I want to solve this properly all at once this time,"
and clearly chose implants.
We respected that decision,
set the most stable long-term treatment direction,
and began treatment in earnest.
Bone grafting is not essential.
We proceed only when it is truly necessary.

Bone grafting
is not required for every implant.
If bone grafting is performed, it increases not only the cost burden
but also the surgical period by 3 to 6 months or more.
There is also discomfort during the recovery process.
So after precisely analyzing the width and height of the bone with CT,

if bone grafting is not necessary,
we do not recommend unnecessary procedures.
In this patient’s case, the CT analysis showed that the amount and density of the remaining bone
were sufficient for implant placement.
So we proceeded by placing the implant immediately on the day of extraction,
without bone grafting.
We check and check again,
perhaps to the point of being overly thorough.
Why the Signal Sent by Protruding Front Teeth and Wobbly Teeth Should Not Be Ignored
Because once an implant settles into place,
it is difficult to correct,
the process of checking the exact position and direction before placement
cannot be overemphasized.
That is why we use a tool called a "parallel pin."
230609
Before starting the actual placement,
it serves as a kind of "compass" that maps out
in advance the optimal path the implant should take.
Thanks to this,
we were able to complete placement at the exact position and angle
without losing our way.

Five months later,
after confirming numerically whether the implant and bone had properly integrated,
we were finally ready to take the impression with a good feeling.
Impressions are faster and more accurate thanks to the intraoral scanner.
Why the Signal Sent by Protruding Front Teeth and Wobbly Teeth Should Not Be Ignored
When people hear "taking an impression,"
in the past it meant putting rubber-like material
into the mouth and enduring it for several minutes
until it hardened.

Keeping the mouth open is hard,
a gag reflex may come up,
and you cannot even swallow saliva...
Many people said, "That was the hardest part," haha.
But these days, thanks to the intraoral scanner,
that discomfort has almost disappeared.
What is an intraoral scanner?
Simply put,
it is a device that photographs the inside of the mouth in 3D.
If you slowly move a small camera,
about the thickness of one or two fingers,
inside the mouth,
the shapes of the teeth and gums
appear as a real-time 3D image
on the computer screen.

Many people are amazed and say,
"Oh, so that’s what my teeth look like."
Especially when discussing the color or shape of prosthetics,
we can look at the screen together and talk,
which makes communication much easier.
This patient’s prosthetic also fit perfectly thanks to the intraoral scanner.

This patient also had an impression taken with an intraoral scanner,
and because the scan was highly precise,
the prosthetic fit well right away on the same day.

Usually, when placing an implant prosthetic,
we need to check the height, bite, and any interference
and make small adjustments,
but in this case there was almost nothing that needed major adjustment.
The color was natural,
and the shape turned out very satisfying.^^

Just because teeth are loose and look like they are protruding
does not mean they must be extracted.
Depending on the condition, we first consider whether they can be saved,
and if that is difficult, implants can provide a stable solution.
Rather than worrying alone,
I recommend getting an accurate diagnosis first.
Thank you for reading this long post today. ^^
