When receiving an implant consultation at a dental clinic,

many people hear that “bone grafting is necessary”

and end up worrying about it.

“Since it adds extra cost and takes longer,

can’t I just skip it?”

It is only natural to feel that way.

Today, I’ll clearly explain the cases in which

bone grafting is truly necessary and those in which it is not,

and through real cases, I’ll explain in detail

why bone grafting determines the success of an implant. ^^

Bone grafting — is it really necessary?

To get straight to the point,

not everyone needs bone grafting.

1. Cases where bone grafting is not necessary

If there is no severe inflammation

in the area where the tooth was extracted,

and if enough jawbone remains to firmly support the implant,

surgery can be performed without bone grafting.

2. Cases where bone grafting is absolutely necessary

These are cases where chronic inflammation

has caused the jawbone to dissolve,

or where a long time has passed since the tooth was extracted,

leaving the bone width narrowed.

Especially if inflammation at the root tip has spread

near the nerve canal, it becomes not an option but a “must.”

A case of a patient who visited because both lower molars were loose

Recently, a patient came to our clinic saying

that both lower molars were loose.

At first glance, it looked fine,

but after taking an X-ray,

the condition was more serious than expected.

1. Lower left molar (#37)

This tooth had previously undergone root canal treatment and had even been reinforced with a post,

but secondary decay had progressed nearly to half of the tooth under the gold crown,

leaving it very weakened.

2. Lower right (#46)

It had been covered with zirconia,

but chronic inflammation at the root tip had spread

and was gradually dissolving the surrounding jawbone.

As the bone was dissolving, of course the tooth

had no choice but to become loose.

A dangerous anatomical detail to watch out for: the inferior alveolar nerve

Looking more closely with a CT scan,

the inflammation at the root tip had spread in a dark area.

Especially, the bone had dissolved near the “inferior alveolar nerve,”

which passes directly below the tooth roots.

The inferior alveolar nerve is a very important passageway

that is responsible for facial sensation.

If this nerve is touched while placing an implant,

there is a risk of paralysis or pain,

so it is a structure that must be handled with great care.

What happens if you try to place an implant forcefully

in a case like this, where the bone near the nerve has completely dissolved?

In trying to avoid the nerve,

the implant cannot be properly anchored into the bone

and ends up almost floating in midair.

It obviously cannot be securely fixed, right~?

That is why bone grafting is necessary.

This patient also first underwent bone grafting

to fill the insufficient space after extraction.

You can think of it as filling the dissolved area with bone graft material

to create a foundation that will firmly support the implant.

At this stage, there is something just as important as bone grafting:

that is, “waiting.”

The transplanted bone needs time to fuse with the patient’s own bone

and harden firmly.

This patient also, after bone grafting,

waited long enough for about 3 months,

and after confirming that bone had formed,

we placed the implant root.

Why isn’t the implant crown placed right away?

You may be wondering why the crown (prosthesis)

is not placed immediately after the root is inserted.

Our natural teeth are not directly attached to the bone;

there is a perfect cushioning system called the “periodontal ligament.”

It absorbs the impact that comes from chewing.

But an implant is directly attached to the bone

without this cushioning system.

What happens if you bite down on hard food before the bone and implant have fully set?

An implant that has not yet hardened

cannot withstand that strong force

and will collapse as it is.

The patient waited about four more months,

waiting for the implant and bone to become firmly united.

After accurately confirming the degree of integration with data rather than guesswork,

we completed the final prosthesis.

Seeing the patient now able to eat comfortably with both molars,

I also felt a great sense of fulfillment. ^^

The standard for treatment is always the patient’s “safety.”

Bone grafting is not simply a process that

extends the treatment period,

but a valuable time to lay the foundation

so it can still be used firmly even 20 years from now.

Dental treatment can be frightening,

and there are many things you may be curious about.

From the patient’s perspective,
