When it comes to implant surgery,
how long does it take?
One of the most common questions
patients ask when they visit the dental clinic
is the implant treatment period.
They often wonder whether the implant can be placed right after tooth extraction,
or whether they need to wait for a long time.
Today, I will explain in detail the difference between immediate placement,
which is done on the same day the tooth is extracted,
and delayed placement,
which is performed after waiting for the gum bone to heal,
through real patient cases.
What is the difference between delayed placement and immediate placement?

To put it simply, it is similar to checking the condition of the ground
when building a house.
- Immediate placement

When there is no inflammation in the extraction site
and the surrounding bone is strong,
this is the method of placing the implant at the same time as the tooth extraction.
It has the advantage of significantly shortening the overall treatment period.
- Delayed placement

This is chosen when there is severe inflammation at the root tip
and the surrounding bone has been greatly resorbed.
First, the inflammation is removed,
and we either wait for the bone to fill in,
or strengthen the foundation through bone grafting
before placing the implant.

Let's take a look at the photos of a patient who underwent delayed placement.
If you look at the right lower back molar area (#47),
the root tip appears black, right?

Inflammation developed at the root tip, and as a result of that inflammation,
the bone resorbed and appears black.
The denser the gum bone,
the whiter it appears,

and when the bone resorbs due to inflammation
and the density decreases, it looks black like this.

Shall we look at the 3D CT?
The area marked in red is the inferior alveolar nerve,
a major nerve responsible for sensation in the lower jaw,
and the yellow area is the space where the bone has been resorbed
due to inflammation.

The most important thing when placing an implant
is to avoid this nerve and place it safely.
If we were to place it immediately in this condition,
the implant root would not be properly fixed in the empty space
where the bone has resorbed.
Also, for the implant to gain support,
a bone length of at least 7–8 mm must be secured,

and in the photo, the distance from the resorbed area to the nerve
is almost touching.
In such cases, forcing an immediate placement can increase the risk
of accidentally affecting the nerve.
So in cases like this,
we first wait for the bone to recover strongly,
and then place the implant once the bone has been stably regenerated.

After removing the inflammation and performing bone grafting,
we waited about two months for the bone to harden firmly.
Once we confirmed that the gum bone had formed stably,

it was placed safely while avoiding the inferior alveolar nerve.
It would be nice to place the restoration right away in this state,
but we need to wait safely until the implant and bone have integrated.

Two months after placement, we confirmed that the implant and bone had
fused completely into one,
and successfully completed the restoration as well.

Now let’s look at the opposite case, where immediate placement was possible.
If you look at the left lower molar (#36),
one side of the root is broken off.

The white structure above the broken root
is a post, called a "post."

It is a post placed to support the restoration
when the crown of the tooth has been severely damaged,
which also means the tooth had become quite weak.
In the end, the patient came to the clinic because the root also broke
while eating hard food.

Let's take a look at the CT.

Did you notice the difference compared with the patient shown earlier?
The inferior alveolar nerve is marked with a red circle.
The bone between this nerve and the root tip is clearly well filled in
and has a white, high-density appearance, right?
In this patient, there was no inflammation at the root tip,
and because only the root had fractured due to weakness of the tooth itself,
the surrounding bone remained very healthy.
When the foundation is this strong,
the implant can be placed immediately on the day of extraction.

Thanks to this, the treatment period could be greatly shortened.

and then took impressions and completed the restoration.

it took exactly two months.
When it comes to implants, stability is more important than speed.

whether an implant can be placed right away depends on
the patient’s gum bone condition and whether inflammation is present.

Everyone wants to finish quickly,
but just as building a house too hastily on weak ground can cause major problems later,
the same applies to implants.
If there is not enough bone, waiting is necessary,
and if the bone is strong, immediate placement can help restore function quickly.
