When most people hear that there is
inflammation at the root tip,
they tend to think,
‘So this tooth is at the end of its life now,’
and start thinking about extraction.

But even if there is a definite alternative
in the form of an implant,
there is clearly still a "chance"
to save your own tooth once more.
Today, I’ll share a case of a molar that had been
recommended for extraction,
and was brought back to life through
"retreatment of the root canal." ^^
Why does inflammation form at the root tip even when there is no pain?

Inside a tooth, there is tissue called the
"pulp," through which nerves and blood vessels run.
If a cavity is deep,
if a previous root canal treatment was not perfect,
or if a tiny crack forms in the tooth and bacteria enter,
inflammation develops at the root tip
(periapical lesion).

To think that there is a large pocket of inflammation
in the root of a tooth that looked perfectly fine,
to be honest, that’s shocking.. ㅜㅜ
What is even more surprising is that
you yourself may not have felt any pain.
Inflammation at the root tip often quietly dissolves bone
without causing pain.

As a result, the surrounding jawbone melts away,
and you eventually reach a situation
where the tooth truly has to be extracted.

Retreatment of the root canal is actually much more difficult
than the initial root canal treatment, and the success rate is lower.
That is why I carefully consider several criteria
before explaining the possibility.

Whether the shape of the inflammation is relatively well-defined
Whether the root itself is not cracked or broken
Whether there is any potential for regeneration even if the jawbone has melted away
If these conditions are met,
I conclude, "Ah, this is not a situation where we need to start with extraction."
A natural tooth never comes back once it has been extracted.
So if there is even a small chance of saving it, ,
I believe that chance should be explained first,
before talking about an implant. ^^
A case of saving a natural tooth with retreatment of the root canal for inflammation in a molar

Not long ago, a patient came in because of a problem with the lower right molar (#37),
which had undergone a root canal a year earlier.

They said they had been advised at another clinic
to have it extracted and then get an implant because of root inflammation, ,

but the patient was still very young.
Too young to get an implant... ㅠㅠ
The CT scan also showed that the root inflammation
had progressed quite a bit,

but because the patient had a strong desire to save the tooth,
we took the time to begin retreatment of the root canal.
For treatment, we removed the prosthetic restoration,
and there was not much tooth structure left, probably because the old cavity had been severe.

After thoroughly removing the old filling material
that had been placed previously,
we carried out several rounds of precise disinfection.

In this process, while sterilizing the inside of the root canal,
we also carefully treated it using a medication called
"Calcipex," which helps encourage the dissolved bone to fill back in.
After a total of four careful disinfection sessions,
we monitored the progress for one month.

One month later,
the black-looking inflammation at the root tip
had noticeably disappeared!!

Now all that remains is to let this tooth heal well
and carefully finish treatment so that the patient can
comfortably eat again. ^^

that has already had a root canal,
there is no need to give up unconditionally.
Of course, not every tooth can be saved,
but if accurate diagnosis and sincere treatment support it,
we can extend the life of the natural tooth once more.
