Depending on the position and structure of the teeth,

orthodontics is not always the right answer,

and in some cases,

veneers should not be done either.

Today, through a case of a patient who was worried about gapped front teeth,

I’ll explain

how the treatment method changes

based on the criteria used.

In technical terms, this condition is called 'diastema,'

and it refers to a state where there is a gap

because the teeth are spaced apart.

It is commonly found in the upper central incisors,

and the causes are more varied than you might think.

• Imbalance between jaw size and tooth size:

When the jaw is too wide compared to the teeth

• Oral habits:

Habits such as pushing the front teeth with the tongue or breathing through the mouth

• Physical structure:

When the line connecting the lip and gums (the upper labial frenum)

is attached abnormally

Rather than simply filling the gap,

it is important to first identify why it opened

so that recurrence later on can be prevented.

That is why precise diagnosis is more important than anything else!

There are three main ways to treat diastema.

1. Orthodontics

A method of directly moving the teeth to realign them

2. Crown

A patient who recently visited our clinic had

a gap between the central front teeth,

and at the same time, the upper left molar (#26)

was missing.

The patient’s biggest concern was the 'treatment period.'

If orthodontics were chosen,

a dental implant could only be done after the full 1 to 2 years of orthodontic treatment were completed.

This is because once an implant is fixed into the bone,

it is difficult to move.

From the patient’s perspective, orthodontics would make the treatment period much too long,

so it was a burden....

After a thorough discussion with the patient,

we decided to quickly improve the four front teeth with veneers,

and proceed with an immediate implant for the molar.

Many people worry that when they hear "veneers,"

"Aren’t they going to shave down all of my natural teeth?"

In fact, the average amount of tooth reduction for veneers is

about 0.3 to 0.7 mm.

That is an extremely small amount, about the thickness of a fingernail.

Unlike crown treatment, which requires shaving the entire tooth all the way around 360 degrees,

because only the outer surface of the tooth is very lightly refined,

the biggest advantage is that as much of the natural tooth as possible can be preserved.

In particular, patients with teeth that are spaced apart,

those with 'diastema,'

are in a rather favorable condition for veneer treatment.

This is because there is already enough space between the teeth.

Rather than forcibly shaving down teeth to create space for the prosthetic,

it may be easier to understand it as the principle of

'filling in' the already empty gap with a more attractive shape. ^^

Thanks to this, tooth reduction can be minimized,

and depending on the situation, it is even possible to do

'no-prep veneers' that require no shaving at all.

There is no rule that veneers should never be done.

In some cases,

they can bring about a truly dramatic change.

However,

whether my teeth are actually a case for veneers,

and what is more efficient when combining treatment with options such as implants,

These are the kinds of questions that should not be approached blindly;

otherwise, veneers done without careful thought will only lead to regret later.

If something should not be done, we should say it cannot be done,

and if orthodontics or a crown would be better,

I believe it is the role of the medical team to say that honestly.
