The reason we visit the dentist is usually because of tooth pain.
In most cases, if cavities or gum disease are detected early,
we can manage the teeth or gums through treatment
and use our natural teeth for a long time.
When a cavity is too deep and progresses to the root,
or when periodontitis, also known as pyorrhea, is severe,
extraction is often unavoidable.
Before implants became widespread, such cases were restored with prosthetic treatment called a bridge.
Recently, however, implants are mainly used to treat only the affected area without preparing the surrounding teeth.
During implant treatment,
one inconvenience is that the implant area must remain as an empty space without a tooth
until the implant becomes firmly integrated with the bone through osseointegration.
This can cause discomfort in daily life.
In the case of molars, eating is somewhat inconvenient,
but since they are not visible, many people can put up with it for 2 to 3 months.
However, in the case of front teeth,
going several months with a missing tooth can be very inconvenient in daily life,
since you still have to live your social life.
In the past, a temporary denture was provided for this area,
but because it is a removable prosthesis, it was somewhat unnatural
and aesthetically less harmonious, which was a drawback.
Recently, when initial stability is good on the day of implant surgery,
a temporary tooth can sometimes be completed the same day.
Is a temporary tooth possible on the day of implant surgery after extraction due to periodontal disease??


The patient visited because the gums around the lower front teeth were often swelling.
The two teeth had spaced apart due to tooth movement caused by alveolar bone loss from periodontitis.


One of the two teeth had progressed significantly with periodontitis, and the adjacent tooth also showed mobility of grade 2 or higher, so both teeth needed to be extracted.
Implant surgery was possible at both tooth positions,
but considering that these were lower front teeth,
we decided to place only one implant and connect an artificial tooth next to it rather than placing two implants.
As mentioned earlier, since treatment after implant surgery can take at least about 3 months to complete,
we fabricated a temporary tooth immediately after surgery so the patient could get through that period.

Before starting the surgery, we request the fabrication of a temporary tooth in advance through an intraoral scanner.
We coordinate with the lab regarding the implant direction and the expected tooth shape,
and then modify the fabricated temporary tooth using a temporary abutment.
For the surgery, the areas with severe periodontitis and significant alveolar bone resorption were treated with alveolar bone grafting to preserve gum volume
so that the gums would not collapse over time,
and a temporary tooth was fabricated through a temporary abutment.


This is the appearance immediately after surgery.
The immediate temporary tooth for a front tooth implant is not for chewing,
but is intended to fill the empty space.
Therefore, we adjusted the bite against the upper teeth and modified the temporary tooth to match the gum margin.
Of course, the color cannot be matched as precisely as with the final prosthesis,
but the patient said it was much more natural than wearing a removable temporary denture.
Time required until completion of the final prosthesis after surgery

It took about 3 months from surgery to the final prosthesis.

The grafted alveolar bone area also maintained good volume,
and the prosthetic line was made naturally.
Compared with the temporary tooth, the color was matched to the surrounding teeth,
and the shape of the tooth was also designed to fit the gums after postoperative swelling had subsided.
Since the front teeth are the most visible when smiling and speaking,
we made several adjustments based on the patient’s requests during the process,
and after the patient finally approved, we completed the final placement.
The axis of the lower tooth is slightly tilted,
and if we had corrected the tooth axis during fabrication and also restored the surrounding teeth together,
the aesthetic completeness would have been higher.
However, the patient wanted only the treated area to be used in a way that matched the existing teeth,
so we finished it as naturally as possible.
The treatment has been completed, and the patient has not been able to visit recently.
However, before treatment, if slight mobility was observed in the surrounding teeth,
after extraction of the tooth that caused the inflammation and completion of gum treatment,
the mobility of the surrounding teeth stabilized, so no additional treatment for other teeth was needed.
Because front teeth are responsible for aesthetic function,
rapid restoration is important when extraction is performed.
If initial implant stability is good, making a temporary tooth like this is the ideal treatment method whenever possible.
It would be good to do this for every case,
but the diagnosis and treatment process can change depending on the condition of the bone quality and the surrounding alveolar bone.
