It is not only middle-aged or older people who undergo lower eyelid surgery.
Elementary school students and people in their 20s also sometimes undergo lower eyelid surgery.
People in their 20s may undergo lower eyelid surgery when having aegyo surgery or when removing aegyo-related foreign materials, and in the case of elementary school students, lower eyelid surgery is performed to correct severe entropion, where the eyelashes irritate the eyeball.
Lower eyelid ectropion

The case below is one in which a lower eyelid surgery performed by an ophthalmology clinic in childhood to correct entropion went wrong, resulting in #하안검외반.

If lower eyelid ectropion is severe, the #삼백안 symptom, where a lot of white is visible below the black pupil, will look more pronounced.
Not all triple-whites are medically abnormal, but they can look cosmetically unattractive.

Also, when you open your eyes straight and forcefully, the interaction between the levator palpebrae superioris and the lower eyelid’s lower eyelid muscle outer layer makes the triple-white appearance worse, so
it becomes difficult to open the eyes straight, making it look like ptosis.
Because of these two effects, lower eyelid ectropion is aesthetically quite undesirable.

Another problem with lower eyelid ectropion is that when looking upward, the ptosis and triple-whites become even more severe.
The whites of the eyes become visible to the point that the eyeballs seem more protruded, and the white area above the black pupil is covered even more.

Even when looking downward, the lower eyelid may appear detached rather than touching the black pupil.

The way to solve all of these problems is #하안검외반교정.
Usually, the reason lower eyelid surgery goes wrong is when the amount of skin and muscle to be removed from the lower eyelid is misjudged.
When tissue becomes insufficient, the lower eyelid is pulled downward and ectropion occurs, so if this underlying cause is addressed, #안검외반교정 is naturally achieved.
It can be solved with the #안검피부이식 method, but
I prefer the #복합조직이식 #compositegraft method using skin and muscle from the upper eyelid, which is most similar to the skin and muscle tissue of the lower eyelid.
This is because if not only skin but also some muscle tissue is grafted, volume can be added to the flat lower eyelid.

Unlike middle-aged patients, lower eyelid surgery in people in their 20s is often performed to remove injected materials or foreign substances used to create fuller aegyo-sal.
In such surgeries, only the foreign material should be removed, but if too much normal muscle tissue or skin is removed, tissue deficiency can occur and ectropion may develop.
The case 바로 below is one in which ectropion developed because too much tissue was removed while removing lower eyelid aegyo filler.

In particular, triple whites were severe on the medial inner corner side, and the lower eyelid was pulled downward, but after lower eyelid skin grafting, the ectropion symptoms improved considerably.

Even when looking upward, the lower eyelid that had been turned outward by ectropion was well attached to the eyeball, although only one week had passed since surgery.

When closing the eyes, even with force, the medial inner corner side had been slightly lifted before, but after the skin graft, the eyes close well.
In middle-aged and older patients, lower eyelid incision surgery to correct under-eye bulging can sometimes lead to ectropion.
Even if not much skin tissue was removed, contraction can occur due to abnormal scar tissue response after the incision, causing the lower eyelid to turn outward.

This is a case where there was no ectropion at all before surgery, but within two weeks after lower eyelid surgery, contraction developed in the incision area and the fat-removed area of the lower eyelid, causing the eyelid to turn outward severely.
The right eye also developed slight ectropion on the medial inner corner side, but since this was an acute case, it naturally corrected as the contraction eased.
In the left eye as well, acute contraction developed under the eye, causing the aegyo area to bulge up abnormally thickly.
Because the ectropion in the left eye was severe and might take too long to resolve naturally, skin was grafted from the ipsilateral upper eyelid.
At this time, if the contracted tissue is properly released and the lacking skin is grafted, the ectropion is well corrected immediately after surgery.
