Today, I’d like to talk about 230-degree three-dimensional zygoma reduction surgery.
I’ve written various columns and uploaded YouTube videos about it in different ways.
Among my patients, some think that 230-degree three-dimensional zygoma reduction surgery is a cure-all,
so I’d like to give a few examples and explain the cases in which 230-degree three-dimensional zygoma reduction surgery is not effective.
Men with large, thick cheekbones
Men whose cheekbones are very large and thick are not suitable for 230-degree three-dimensional zygoma reduction surgery.
230-degree three-dimensional zygoma reduction surgery is a procedure that makes a partial osteotomy of the 45-degree cheekbone area and pushes the cheekbone inward.

In men with thick bone, there are cases where the bone does not move no matter how well the partial osteotomy is done.
Even if the 45-degree cheekbone area is shaved and a partial osteotomy is performed, if the bone is too thick, it will not move inward.
Therefore, for these patients, we review the CT scan in advance and explain 3D cheekbone reduction surgery.
Regardless of whether the surgery will be effective, since the surgery itself may not be performed accurately, it seems better not to consider 230-degree three-dimensional zygoma reduction surgery.
Of course, among men, there are many who do not have thick bones.
Because large cheekbones do not necessarily mean thick bones, it is better to have a CT scan and receive a consultation.
People who want to reduce the area of the 45-degree cheekbone
230-degree three-dimensional zygoma reduction surgery does not remove any bone.
If you look at this CT scan,

In 3D cheekbone reduction surgery, the bone in the blue area is removed.
Therefore, the length of the red area becomes shorter.

When the face is viewed from the front, if the red line appears wide and the face looks large, 3D cheekbone reduction surgery is needed to reduce the 45-degree cheekbone bone.
However, 230-degree three-dimensional zygoma reduction surgery does not remove bone; it is a procedure that lowers the height of the 45-degree cheekbone, so it is not suitable for people who want to reduce the area of the 45-degree cheekbone.
In general, many men have a large width in the 45-degree cheekbone area, so it may be helpful to know this in advance before surgery.
People who want to reduce the cheekbones as much as possible
230-degree three-dimensional zygoma reduction surgery reduces cheekbone volume less than general 3D cheekbone reduction surgery.
The amount of reduction in the side cheekbones is the same, but because there is no bone removal in the 45-degree cheekbone area and the bone is shaved rather than pushed inward, you can think of the reduction in the 45-degree cheekbone area as being about 20–30% less.
Therefore, if your goal is to reduce the cheekbones as much as possible through surgery, it is better to consider general 3D cheekbone reduction surgery rather than 230-degree three-dimensional zygoma reduction surgery.

In fact, 230-degree three-dimensional zygoma reduction surgery has many advantages.
I previously mentioned its advantages through a column, so I hope you’ll take a look.
Despite these various advantages, if you fall into the three cases mentioned above, I can say that it is better not to consider 230-degree three-dimensional zygoma reduction surgery.
Each patient has a different cheekbone shape and a different desired outcome.
Rather than thinking that one surgery is the best, I believe that carefully weighing the pros and cons can lead to more satisfying results.
