Is revision surgery possible with 230-degree three-dimensional cheekbone reduction surgery?

After cheekbone reduction surgery, some people want revision surgery again because of various concerns.

Today, we’re not talking about revision cheekbone surgery under general anesthesia, but rather a simple procedure that can also be done with 230-degree three-dimensional cheekbone surgery.

Let’s look at in which cases revision surgery with 230-degree three-dimensional cheekbone surgery is possible.

230-degree three-dimensional cheekbone reduction surgery is performed through a scalp incision.

Because there is no intraoral incision, not all cheekbone revision surgeries can be done with 230-degree cheekbone surgery.

So in what cases is it possible?

The first and simplest type of revision surgery is for patients who had 3D cheekbone surgery as their first operation, but the arch area was not brought in.

It is likely most common in cases where the surgery was done a long time ago and the back portion was not fixed with pins, allowing it to return to its original position.

Or, in cases of non-fixation surgery, often called quick cheekbone surgery, the back cheekbone often returns to its original position.

In this case, even if 3D cheekbone surgery was performed, revision surgery with 230-degree three-dimensional cheekbone reduction is possible as long as the pin is not positioned too high.

A partial osteotomy is carefully performed along the existing osteotomy line while viewing it with an endoscope.

In the post-op photos, you can see that the side cheekbone has been reduced well.

Also, in cases of patients who had quick cheekbone surgery but saw no effect at all, a partial osteotomy is carefully performed with an endoscope while avoiding the existing osteotomy line.

If partial osteotomy is performed while carefully checking the osteotomy line, good results like the ones below can be achieved.

The second case in which 230-degree three-dimensional cheekbone surgery can be used is shaving of the 45-degree cheekbone.

Of course, if the fixation pin from the 45-degree cheekbone has been removed, the height can be lowered simply by shaving the 45-degree cheekbone through a scalp incision.

But the difficult case is when the existing pin is still present. In that case, only the areas without pins are carefully shaved using an endoscope.

This patient had cheekbone surgery and then had bone graft material applied at a dental hospital.

The patient wanted the bone graft material removed, but did not like an intraoral incision.

In the preoperative CT, you can see that a lot of the graft material has been placed.

In the endoscopic photos taken during surgery, the graft material and the pin can be seen.

After carefully shaving away the graft material, you can see that the height has been greatly reduced after surgery.

You can also see that it has disappeared cleanly on the CT scan.

Of course, if there is a fixation pin, there are limits to shaving.

In most cases, I recommend removing the pin through an intraoral incision and then performing the shaving. In that case, you can expect results like this.

230-degree three-dimensional cheekbone surgery is not a cure-all procedure.

However, for some patients, there are roughly two cases in which it can be done more simply without a full revision surgery.

If you had cheekbone surgery but feel that something is still lacking, it is often because the surgery was not done precisely.

If you get a consultation and simple revision surgery is possible, you may not need to worry about undergoing a difficult operation.
