Recently, a YouTube video featuring actress Jeon Won-ju became a hot topic.
In the video, explanations were given about various surgeries such as a facelift, neck lift, upper and lower blepharoplasty, and fat grafting, along with a high-cost surgical estimate, and Jeon Won-ju’s surprised reaction was captured impressively. From a doctor’s perspective, that scene was entirely understandable.
A facelift is not simply a wrinkle-improvement surgery; especially for elderly patients, it is a high-difficulty procedure that requires even more careful judgment.
Considering that she is 86, her facial condition is unexpectedly good

Looking at Jeon Won-ju’s face objectively, the overall sagging is very minimal for someone who is 86 years old.
The typical indications for a facelift are not clearly present.

Rather, the main signs of aging are relatively clear. The key issue is aging around the eyes: the upper eyelids and lower eyelids.
In this case, a facelift is an excessive choice. With this pattern of aging, there is not enough medical basis to proceed with a facelift.
A more reasonable approach would be as follows.
This combination alone can make the impression look much sharper and produce a more youthful effect.
Not every patient needs a facelift.
This is the principle I emphasize most in the clinic. Everyone has a different facial structure, the direction of aging differs, and the areas and speed of sagging also vary.
Therefore, the surgical plan must always be individualized.

In actual consultations, I often say the following.
“This surgery is not really necessary.”
“The burden is large compared to the effect.”
“This alone is enough.”
That is more reasonable and safer for both the doctor and the patient.
Of course, there are exceptions even in their 80s.
Very rarely, there are cases in which the entire face has severely collapsed even in the 80s, and the jawline and neck contour have completely disappeared.
In such cases, a facelift may be considered after sufficient evaluation and risk analysis.
However, that does not apply to Jeon Won-ju.
