Seoul full-mouth implant dental clinic that does a great job: Why can severe periodontitis even lead to crossbite?
(Full-mouth implant case for periodontitis)
Hello.

#ClinicThatDoesImplantsWell This is Gangnam White Dream Dental Clinic.
Teeth do not simply handle the chewing function of breaking food into smaller pieces.
They also play an important role in making speech clear and maintaining the shape and aesthetics of the face.
Especially from the side view, the teeth and the jawbone that supports them prop up the lips and cheeks,
and they also help maintain the length of the lower face.
So when you look at people who have lost many teeth,
you can often see their lips or cheeks look sunken in,
or they appear older than they actually are.

When multiple teeth are lost due to the main causes of tooth loss, such as periodontitis or trauma,
the remaining teeth lose their positions and shift,
gradually causing the bite to become misaligned.
In severe cases, the lower teeth may protrude farther forward than the upper teeth,
or even a crossbite, where the lower teeth cover the upper teeth, can occur.

Today, I’d like to introduce a treatment case of a patient whose bite became misaligned after many teeth were damaged and lost due to periodontitis.
Because multiple teeth were lost and worn down due to periodontitis,
the condition of the remaining teeth was also very poor.

Because the jawbone had severely resorbed due to inflammation,
the weakened teeth tilted or spread apart,
disrupting the occlusal relationship between the upper and lower teeth and eventually causing a deformity into an abnormal bite, crossbite.

▲ This is the patient’s X-ray before treatment.
The upper and lower teeth were in such a severely worn and damaged state that they were no longer usable,
and the abutment teeth connected by the lower long bridge
showed extensive bone resorption due to severe periodontitis.

To briefly mention the long bridge,
this refers to connecting more teeth with a prosthetic than a regular bridge (3–4 units).

▲ The number of bridge teeth commonly treated in general
When multiple teeth are connected with a long prosthesis,
it has the advantage of restoring the missing area all at once and lowering treatment costs,
but the load placed on the abutment teeth increases accordingly, which is not good for long-term stability.
In addition, since multiple prosthetic teeth are joined into one piece, it is unhygienic.
Food very easily gets trapped under the prosthesis, and this area is difficult to clean properly with regular brushing.

▲ The area treated with a long bridge
shows more severe gum inflammation than the other teeth,
and the surrounding jawbone destruction has progressed more extensively.
That is why patients must carefully manage it using tools such as interdental brushes, Waterpik devices, and floss,
but since very few people manage it that thoroughly,
a lot of people with long bridges also have severe secondary cavities or gum disease.

▲ Removed long bridge and inflamed tissue
For this reason, the patient
underwent implant treatment after extracting all of the remaining teeth in the full mouth.
Compared with the lower jaw, the upper jaw has lower bone density and takes a little longer to heal,
so surgery began with the upper jaw.

The full-mouth implants were placed in a total of two surgeries, starting with the upper jaw and then the lower jaw.

Areas with severe bone loss due to inflammation were avoided,
and implants were placed in areas where relatively more jawbone remained.
This not only allows stable initial implant fixation,
but also reduces unnecessary bone grafting, which eases the burden on the patient.
It also has the advantage of promoting smooth healing at the surgical site,
which can shorten the overall treatment period.
After about 4 to 5 months of implant and jawbone healing,
the placed implants become firmly integrated with the jawbone,
gaining enough stability to allow the patient to eat soft foods.

Now, we move on to the treatment stage where we set the tooth-shaped prosthetics on top of the implants
and restore lost chewing function and aesthetics. ^^

The total treatment period was 6 months.

You can see that the sunken appearance of the lips or cheeks caused by missing teeth has improved,
and the length of the lower face and facial contour have been restored close to normal.^^

In addition, after treatment, the lost teeth moved back into their proper positions,
and the crossbite was restored to a normal bite.

A full-mouth implant is not simply about replacing the number of teeth.
It also corrects chewing function and bite balance,
so it is a treatment that can improve overall oral health and aesthetics together.
The biggest change patients often feel after full-mouth implant treatment is
confidence.
Because the discomfort felt every time they chew,
and the anxiety about their teeth (or dentures) falling out, disappear,
many people also regain confidence when interacting with others.^^

If you are experiencing discomfort because your bite has become misaligned due to tooth loss, as in today’s case,
please confirm an accurate diagnosis and treatment plan through a consultation with a specialist.
You can 충분히 regain a healthy smile and comfortable mealtimes.^^
