Have you perhaps been told that you need to have your wisdom teeth removed during orthodontic treatment?

Although you may not have wanted to remove them, situations often arise during the alignment process where extraction becomes necessary.

It can feel uncomfortable to remove a tooth that seems fine.

However, if there is not enough space, or if it is interfering with tooth movement or eruption,

then tooth extraction can also play an important role in treatment.

Looking at the panoramic image taken at the dental clinic in Ogeum-dong,

all four wisdom teeth are present.

This case was scheduled for orthodontic treatment.

Orthodontics is not simply about aligning the teeth,

but about creating a functional bite and a stable dental arch structure.

When non-extraction orthodontics is desired, space is often created by moving the teeth backward.

However, if wisdom teeth are present, they can interfere with treatment,

so in some cases all of them are removed even if they have not erupted.

At the Ogeum-dong dental clinic, we planned to remove the two lower wisdom teeth first before starting orthodontic treatment.

When teeth are impacted, they are often located deep within the gums and jawbone,

so it is difficult to clearly understand their relationship with the surrounding nerve structures using only a simple panoramic image.

Therefore, it is necessary to precisely analyze the anatomical structures in 3D through CT.

The teeth located at the back often overlap with or are very close to multiple structures,

so this is an essential step to prevent unnecessary damage to nerves and tissues.

At the Ogeum-dong dental clinic, we first examined the right side in order to proceed with the lower teeth.

Although it was impacted,

it was not especially close to the structures that needed caution,

and it was judged that it could be removed safely.

It could be removed without much difficulty,

but the important part was the lower left side.

Looking at the CT, the occlusal surface (the upper surface of the tooth) was horizontally impacted toward the lingual side.

Because the lingual mucosa is especially close to nerves and blood vessels, extra care is needed during incision and dissection.

Attempting to approach it too aggressively can cause side effects such as decreased sensation.

Therefore, at the Ogeum-dong dental clinic, after carefully analyzing the position,

we approached it with a safe incision method

and proceeded carefully through delicate dissection.

As a result, it was able to be removed safely,

and both areas recovered without major pain or side effects.

Currently, the area marked on the upper jaw is not erupting normally,

and we are currently monitoring the progress using an eruption guidance device.

If, during the process of promoting eruption, the adjacent wisdom tooth is judged to be physically interfering,

additional measures will be taken if necessary.

Removing a tooth does not cause the same pain for every patient.

The simpler the extraction process is and the more tissue damage is minimized,

the more the surgical burden can be reduced and recovery can be faster.

It can be part of a comprehensive treatment process that considers not only simple extraction,

but also alignment and oral health.

So instead of thinking of it only as something scary and inconvenient,

it may be better to think of it as an approach for safe and reliable treatment.
