This is Yonsei Dagam Dental Clinic in Songpa-gu.
Among the anatomical structures of our face,
there is a sinus called the maxillary sinus.
It is an air-filled cavity
located between the cheekbone and the upper jawbone,
and when inflammation develops here,
it becomes maxillary sinusitis, a type of sinusitis.
In the past, to address chronically recurring
sinusitis problems,
a method called the Caldwell-Luc Operation
was commonly performed.
It may sound difficult just from the name, but
in simple terms at a Songpa-gu dental clinic,
this procedure involves creating access to the maxillary sinus
by opening the anterior wall between the oral gum tissue and the maxillary sinus,
and through this opening,
removing inflamed tissue inside the sinus
and promoting drainage.
However, less invasive methods such as
endoscopic sinus surgery are now preferred,
so patients who previously underwent
a Caldwell-Luc Operation
are not commonly seen in clinical practice.
Although it is not a common case,
patients who had this surgery in the past have
typical anatomical features,
which we would like to look at together today
at our Songpa-gu dental clinic.

In the case corresponding to the above description,
when we look at the area marked in the photo,
severe inflammation was progressing around the roots
of the right upper molar area.
The supporting bone had been lost due to inflammation,
causing the teeth to become loose,
and there was a state of severe bad odor and pus drainage.
On closer inspection,
one of those molars also showed
a fractured root portion.
The tooth was no longer salvageable,
and recovery with an implant after extraction
was necessary.
However, because this area is close to the root region
and the maxillary sinus,
it is an important factor to consider
during treatment at a Songpa-gu dental clinic.
In patients who have undergone
a Caldwell-Luc Operation,
there is a typical feature in which a defect occurs
in the maxillary sinus bony wall.
This is because the intentionally created space
no longer closes completely
and often remains with partial bone defects.
If the bony wall remains lost
for a long period of time,
the lack of bone needed for implant placement
can make treatment difficult.
A key factor in implant treatment
is securing initial stability.

Therefore, in such cases, CT analysis is used
to accurately identify in advance the bone that can be used,
and if necessary, additional bone grafting
must be performed together.

At the Songpa-gu dental clinic, we examine the structure
three-dimensionally from various angles like this
and assess the extent of the inflammation.

We also carefully planned the angle and depth
to avoid the defective area and place the implant
in healthy bone tissue as much as possible,
and selected an artificial root (fixture)
of the appropriate size and length.
After a thorough analysis,
the problematic molars were first extracted.
Then, drainage was performed
to meticulously remove the inflammation.

After that, the area was disinfected
to create a clean environment,
and the artificial root was placed
in the optimal position planned in advance
through analysis.
As a result, the surgery was completed safely
with good initial stability.
Afterward, through inflammation control
and regular checkups,
management was carried out thoroughly, and

the final prosthesis setting was completed stably.
At the Songpa-gu dental clinic, the severe bad odor
and pus problems that were present before treatment
were improved.
Even now, the patient continues to receive regular checkups
and preventive care, with thorough follow-up management,
and the recent checkup results confirmed that
the implant is being maintained well
without any problems.
As such, although it is not a case commonly encountered
in clinical practice,
even in cases with a past surgical history such as
a Caldwell-Luc Operation,
safe implant treatment is possible
through thorough analysis and planning.
What matters most is accurately understanding
each person's unique anatomical situation,
and choosing a customized approach
that fits it.
