There are several ways to restore a missing tooth area.
Implants, bridges, and partial dentures all have different advantages and disadvantages, and the best choice varies from patient to patient.
Among these, partial dentures are relatively minimally invasive
and tend to be less costly.
However, many people think that you can simply make one and wear it,
but in fact, many factors must be considered,
and I would like to explain them in this post.
- Remaining roots and decay beneath crowns

First, if we look at the preoperative photos,
the lower teeth are fractured due to decay.
The crowns are already damaged and no longer remain,
and in some areas only the tooth roots can be seen.
When the crown is gone and only the root remains,
this is called a remaining root.
Since there is no crown, it cannot function,
and remaining roots below the gum line can become
a source of inflammation and infection,
so extraction is unavoidable.
What about the other areas?

the marginal fit between the crown and the gum
is assessed to be compromised.
Although it may appear hidden by the crown from the outside,
the root is actually exposed as is,
and decay is progressing through that area.
If the decay has progressed deeply,
even if the crown is restored,
the prognosis is likely to be poor.

In such cases, implants can be considered, but
because multiple implants would be needed,
the cost may be burdensome.
If the patient is 65 years of age or older,
insurance-covered implants may be applicable,
but since the benefit is limited to
a maximum of 2 per person for life,
if several are needed,
out-of-pocket costs will eventually be added.
For those who are concerned about the financial burden,
a partial denture (RPD) can be chosen.
However, one point the Ogeum Station Dental Clinic would like to note is that
RPDs secure retention by engaging clasps on the remaining natural teeth.
But what happens if the condition of the remaining abutment teeth is poor?
Before long, additional extractions become necessary,
and the prosthesis must be remade.
That is why, when planning treatment,
it is important to carefully evaluate the prognosis,

and if a tooth will be difficult to maintain long term,
it should be approached from the start with extraction in mind,
in order to improve safety.
- The structure that creates stability
For a partial denture to seat properly,
the contact surface with the teeth must be precisely designed.
What is needed at this time is a surveyed crown.
This is a crown whose outer shape is intentionally designed
so that the clasp can engage accurately.
As shown earlier by the Ogeum Station Dental Clinic,
even if existing implant prosthetics are present,
there is no structure for the clasp to engage,
so a new one must be made.

In addition to the surveyed crown,
another key element of stability is
the rest seat.
This is a concave groove made on the occlusal surface of the abutment tooth,
where the rest of the partial denture seats.
This structure is necessary to distribute occlusal forces
and prevent the denture from lifting up and down or moving unnecessarily.
The position and shape of this rest seat
can vary depending on the design.

For example, an interproximal occlusal rest seat
is a groove created at the contact area between two teeth,
which can provide greater retention.
However, in this case, the prosthetics for both teeth
must be remade.
In the lower left area, making an interproximal rest seat
seemed to be the best option, but to proceed this way,
all of the implant prosthetics in the lower left area would have to be removed
and remade.
The patient found the process of replacing all the lower left prosthetics
distressing, so even though some retention would be sacrificed,
a rest seat was made on only one tooth.
- Precise design creates long-term results

With prosthetics custom-made to suit the individual,
aesthetics and function are restored.
In this way, a partial denture (RPD)
is not simply a prosthesis that fills a missing space.
It requires precise design so that the oral condition can be accurately understood
and a good prognosis can be created.

Only when all of the steps explained earlier by the Ogeum Station Dental Clinic
are properly and completely followed
can a prosthesis be completed that can be used comfortably for a long time.
If treatment is rushed or skipped,
eventually the situation will require retreatment,
and more time and money than before will be needed.
I think everyone wants to reduce the financial burden.
That is precisely why getting the initial design right
is even more important.
